RE: APPROVE EXTENSION AGREEMENT FOR NEIGHBOR-TO-NEIGHBOR PROGRAM
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Extension Agreement for the
Neighbor-to-Neighbor Program between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and the Colorado Department of Human Services, with terms and conditions being as
stated in said extension, and
WHEREAS, after review, the Board deems it advisable to approve said extension, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Extension Agreement for the Neighbor-to-Neighbor Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the
Colorado Department of Human Services be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said extension.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 4th day of May, A.D., 1998.

EXTENSION AGREEMENT
Re: Contract dated July 14, 1997 Contract Routing No. 98-IHA-01223 between the
Department of Human Services, Division of Child Care, 1575 Sherman Street, 1st
Floor, Denver, CO 80203-1714, referred to as State, the Weld County Board of County
Commissioners for the use and benefit of the Weld County Division of Human Services,
1551 North 17th Avenue, Greeley, CO 80632, referred to as Contractor, and relating to
services for recruitment and training of welfare recipients as child care providers.
1. Authority exists in the Law and Funds have been budgeted, appropriated, and
otherwise made available and a sufficient unencumbered balance thereof remains
available for payment in Fund Number 100 Appropriation Code 462 and Contract
Encumbrance Number SC IHACCLC9801223.
2. Required approval, clearance, and coordination has been accomplished from and
with appropriate agencies.
3. The period of performance for the Contract shall be extended to December 31,
1998.
4. All other provisions set forth in the contract shall remain the same.
5. A copy of the Contract is attached hereto as Exhibit 1.
6. The effective date of this Extension Agreement shall be June 30, 1998.
7. Except for the Special Provisions, in the event of any conflict, inconsistency
variance, or contradiction between the provisions of this Extension Agreement and any
of the provisions of the Contract, the provisions of this Extension Agreement shall in all
respects supersede, govern, and control. The Special Provisions in the Contract shall
always be controlling over other provisions in the Contract or Extension Agreement.
The representations in the Special Provisions concerning the absence of bribery or
corrupt influences and personal interest of State employees are presently reaffirmed by
the signatories hereto.
8. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT
FISCAL YEAR ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING
APPROPRIATED, BUDGETED, AND OTHERWISE MADE AVAILABLE.

9. THIS EXTENSION AGREEMENT SHALL NOT BE DEEMED VALID UNTIL IT
SHALL HAVE BEEN APPROVED BY THE CONTROLLER OF THE STATE OF
COLORADO OR SUCH ASSISTANT AS HE MAY DESIGNATE.
10. Dated this 16th day of April, 1998.

on the date above written.
