RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
WITH WELD MENTAL HEALTH CENTER, INC., AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and the Weld Mental Health Center, Inc., commencing June 1, 1996, with further
terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to Child
Protection Agreement for Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and the Weld Mental Health Center, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 22nd day of July, A.D., 1996, nunc pro tunc June 1, 1996.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND THE WELD MENTAL HEALTH CENTER, INC.
THIS AMENDMENT made and entered into this 1st day of July, 1996, by and between the
Board of Weld County Commissioners, on behalf of the Weld County Department of Social
Services, hereinafter referred to as "Social Services," and the Weld Mental Health Center, Inc.,
hereinafter referred to as "Mental Health."
WITNESSETH
WITEREAS,the parties desire to modify their current contract for child protection services
(FY96-12000) in order to expand the scope of services; and
WHEREAS, the authority exists in the Law and Family Issues Cash Funds have been
budgeted, appropriated and otherwise made available; and
WHEREAS, required approval, clearance, and coordination have been accomplished from
and with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree
as follows:
1. This Amendment shall be effective June 1, 1996.

3. Exhibit A, Scope of Services, shall be revised as follows:
L Psychological Evaluations
A. Target/Eligible Population
The population to be served under this contract includes abused and neglected
children who are at risk of out-of home placement, who are clients of the Weld
County Department of Social Services, and are in need of mental health services.
The family members of these children and adolescents will also be eligible for
services.
B. Types of Services to be Provided
Services to be provided under the terms of this contract include the following:
1. Family/Child/Adolescent Psychological Evaluation/Assessment Services
Families, children, and adolescents who are eligible and in need will be
provided screening, evaluations, and other assessment services intended to
provide information needed by the staff of Social Services. The evaluation
procedures will be designed to produce useful responses to specific referral
questions mutually developed by the Licensed Psychologist and Social
Services'Caseworker assigned to the case. The parties will also collaborate
contacts that need to be made prior to the completion of the evaluation.
The Mental Health will provide Social Services a written report upon
completion of the assessment which clearly states methods used, contacts
made, tests administered, results, and recommendations relevant to the
referral questions. The content of this assessment will be made available to
the family and other relevant parties, subject to the American Psychological
Association ethical standards and other legal considerations.
2. Consultation
A Licensed Psychologist of Mental Health will be available to directly
consult with caseworkers of Social Services regarding the integration of
assessment results into the overall care plans for children, adolescents, and
families. This may include issues pertaining to the evaluation of suicidal
clients and the need for inpatient placement or other levels of intervention.

The Licensed Psychologist would be available to assist in obtaining written
feedback from Mental Health clinicians regarding the rationale for decision
on these issues.
3. Court Testimony
The Licensed Psychologist providing the above services will be available for
court testimony when given proper notification.
C. Service Objective
The objective of the contract is to improve services for clients and strengthen the
impact that the Social Services administration in Weld County will have in carrying
out its mission and purpose.
D. ' Service Time Frames
The turn-around time on evaluations will be no more than forty-five (45) days from
the point of referral.
Consultation services will be as needed to the fullest extent possible, as capacity
allows.
E. Measurable Outcomes
This design will shorten the time required to obtain evaluation results following a
Social Service referral. The services provided by this contract will also increase the
Social Service staff access to the evaluation practitioner when questions about
implementation of evaluation results occur. A greater frequency of contacts between
the evaluation practitioner and the Social Service staff will occur. Faster and more
timely responses form the Children and Family Services Outpatient Program of
Mental Health will occur and therapeutic interventions will be better coordinated
between agencies. All of these outcomes can be quantified and compared with status
prior to the implementation of the contract. The Directors of the Department of
Social Services and the Mental Health Center shall jointly assign measures that will
serve as specific outcome targets.
- Emergency consultations will occur within twenty-four(24) hours of request.
- All psychological evaluations will have clear recommendations and methods

that will be accepted by the court.
Five (5) to six (6) assessments will be accomplished each month.
F. Workload Standards
Five (5) to six (6) assessments of varying types will be accomplished each month, as
dictated by demand
G. Staff Qualifications
The Mental Health staff performing these assessments will be a Licensed
Psychologist with appropriate training and expertise with formal evaluation
techniques.
IL Services for Adoptive Parents
A. Target/Eligible Population
The population to be served under this part of the contract are adoptive parents of
special needs, emotionally disturbed, and/or behaviorally difficult to manage
children.
B. Types of Services to be Provided
Services to be provided under the terms of this contract include the following:
1. Facilitation of an adoptive parent support group to meet at least once per
month for the purposes of discussing, managing, and resolving problems that
adoptive parents need assistance with.
2. Provide individual consultation to adoptive parents at least twice a month
regarding the specific problems they are having with individual adoptive
children and/or their family.
C Service Time Frames
Services for referred parents will begin within seven (7) days from the date of
referral. If this time frame cannot be met, Mental Health will notify Social Services
within seven (7) days.

D. Measurable Outcomes
Adoptive parents participating in this service will gain greater knowledge about their
role as adoptive parents, demonstrate greater confidence in their role as adoptive
parents, and be able to identify and manage problems common to adoptive families.
E. Workload Standards
To be determined by the number of referrals and participants.
F. Staff Qualifications
The Mental Health staff involved in this service will be knowledgeable and
experienced in adoptive placement, family system issues common to adoptions,
mental health issues of children, attachment/bonding theory, and practical problem
solving regarding all facets of adoptive families.
III Services for Foster Parents
A. Target/Eligible Population
The population to be served under this part of the contract are foster parents of
special needs, emotionally disturbed, and/or behaviorally difficult to manage
children.
B. Types of Services to be Provided
Services to be provided under the terms of this contract include the following:
1. Facilitation of a foster parent support group to meet at least once per month
for the purposes of discussing managing, and resolving problems that foster
parents need assistance with.
2. Provide individual consultation to foster parents at least twice a month
regarding the specific problems they are having with individual foster
children and/or their family.

C. Service Time Frames
Services for referred parents will begin within 7 days from the date of referral. If
this time frame cannot be met, Mental Health will notify Social Services within 7
days.
D. Measurable Outcomes
Foster parents participating in this service will gain greater knowledge about their
role as foster parents, demonstrate greater confidence int heir role as foster parents,
and be able to identify and manage problems common to foster families.
E. Workload Standards
To be determined by the number of referrals and participants.
4. Exhibit B, Payment Schedule, shall be revised as follows:
Psychological Evaluations
A. Funding and Method of Payment
Social Services agrees to reimburse to Mental Health in consideration for the work
and services performed under Exhibit A, Provision I, a total amount not to exceed
Forty-three Thousand Seven Hundred Seventy-two Dollars ($43,772.00) under Fund
Code 1888 and Object Code 104.
Expenses incurred by the Mental Health, in association with said project prior to the
term of this agreement, are not eligible Social Services expenditures and shall not
be reimbursed by Social Services.
Payment pursuant to this Contract, if Family Issues Cash funds, whether in whole
or in part, is subject to and contingent upon the continuing availability of Family
Issues Cash funds for the purposed hereof. In the event that said funds, or any part
thereof become unavailable as determined by Social Services, Social Services may
immediately terminate this Contract or amend it accordingly.
B. Fees for Services
Social Services agrees to pay Mental Health for the costs of services as follows:

C. Submittal of Vouchers
Mental Health shall prepare and submit monthly the itemized voucher and certify

made were pursuant to the terms and conditions of Exhibit A, Provision I
II. Services for Adoptive Parents and Services for Foster Parents
A. Funding and Method of Payment
Mental Health agrees to provide, at no charge, to Social Services for services for
adoptive parents and services for foster parents. The participants of these services
will be only Social Services referrals.
Mental Health agrees to set aside a total amount not to exceed Forty-three Thousand
Seven Hundred Seventy-two Dollars ($43,772.00) of Family Issues Cash Funds for
the express purpose of providing services for Social Services adoptive parents and
foster parents.
B. Submittal of Report
Mental Health shall prepare and submit monthly an itemized voucher and certffj'that
the services authorized were provided on the date indicated and the charges made
were pursuant to the terms and conditions of Exhibit A, Provisions II and III.
5. Except as modified herein, the terms and conditions of the original agreement
(FY96-12000) and modifications to the original agreement (FY96-12000),
together with all attachments of the original agreement,remain in full force and
effect.

RE: Amendment to Child Protection Agreemeti for SYrvices Between the Weld County Department
of Social Services and the Weld Mental Health Center, Inc.
Enclosed for Board approval is an amendment to the Child Protection Agreement between the
Weld County Department of Social Services and the Weld Mental Health Center, Inc.
The major provisions of the Amendment are as follows:
1. Revises the scope of services to include support groups and individualized consultation
services, as provided by Weld Mental Health, for adoptive parents and foster parents of the
Weld County Department of Social Services.
2. Clarifies the method of payment. Weld Mental Health will not charge the Weld County
Department of Social Services for such services. Weld Mental Health will access $43.772 from
the Colorado Department of Human Services for the express purpose of providing services to the
Weld County Department of Social Services.
