HomeMy WebLinkAbout951454.tiffRESOLUTION RE: APPROVE WELD COUNTY FAMILY SUPPORT NETWORK FAMILY RESPONSIVE SERVICE AGREEMENT WITH AREA AGENCY ON AGING AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Family Responsive Service Agreement between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Area Agency on Aging, and The Weld County Family Support Network, commencing July 1, 1995, and ending December 31, 1995, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Family Responsive Service Agreement between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Area Agency on Aging, and The Weld County Family Support Network be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said agreement. Webster 951454 HR0065 THE WELD COUNTY FAMILY SUPPORT NETWORK FAMILY RESPONSIVE SERVICE AGREEMENT Agreement made this as co —day of IA (-� , 19 41, by and between, The Weld County Family Support Network, to tedt 814 9th Street, Greeley, CO, 80634, hereinafter known as "FSN," and Weld County Area Agency on Aging, an elder care referral agency located at 1551 N. 17th Ave. , Greeley, CO, hereinafter known as "AAA." Whereas, FSN contracts with employers to provide elder care referral assistance by qualified elder care counselors familiar with Weld county and Whereas, the AAA has such qualified counselors on its staff, and Whereas, FSN and the AAA have discussed the Scope of Services (Exhibit 1 & 2), now Therefore the parties have agreed as follows: 1) TERM: This agreement extends for a period of six (6) months, commencing July 1, 1995 and terminating December 31, 1995, unless otherwise terminated pursuant to the terms of this agreement. Both parties shall have the option to extend this contract for an additional one (1) year period at a price to be negotiated and agreed upon by both parties thirty (30) days prior to the expiration of the Agreement. 2) SERVICE: The AAA will provide the scope of consistent services as described in Exhibit 1 & 2 of this agreement. 3) NOTICE OF EMPLOYEES TO BE SERVED: The service will be provided to employees of the clients who have contracted with FSN for this service. Both parties recognize that new clients will be added from time to time. FSN will use its best efforts to provide a minimum of 30 days notice prior to the effective date of service for any new client. 4) PAYMENT FOR SERVICES: FSN will pay the AAA the amount of $10.00 for each basic referral unit of service performed (Exhibit 1) , and $75.00 for each enhanced referral unit of service performed (Exhibit 2) . Payment for such units of service will be made after the Referral Case Record form (Example Attached) is completed and filed and FSN receives a copy of the Billing Record. Units of service reported on a Billing Record received by the 15th of each month will be reimbursed to AAA on the 30th of that month. Billing Records received after the 15th of the month will be reimbursed on the 30th of the following month. Additional units of service stipulated in Exhibit 2 may be contracted for on an individual basis according to the attached fee schedule. Page 1 of 7 951.151 5) MATERIALS SENT TO FAMILIES: As per the Scope of Service outlined in Exhibit 1, The AAA agrees to the following standards when sending referrals or correspondence to client families: A. Materials will be sent on the FSN Confirmation Letter and Referral Form (Examples Attached ). B. All referrals will be printed or typed and will include identifying as well as descriptive data. C. Materials will be current and up-to-date. 6) QUALITY MEASUREMENTS: AAA & FSN will conduct Quality Assurance reviews of 10% of its clients utilizing basic contracted services and each client utilizing the enhanced contracted resource and referral services. The FSN will receive the original surveys from clients, compile information and report to AAA on a semiannual basis regarding these Quality Assurance measures. 7) FAMILY COMPLAINT PROCEDURES: The AAA agrees to maintain a preapproved written set of procedures for handling caregiver or elder complaints or concerns regarding elder care providers and to have copies of complaints available for viewing upon request by the FSN. If a caregiver or elder reports a potential licensing or registration violation, the AAA must follow the preapproved written complaint procedures. 8) REGULAR REPORTING: The AAA conducts periodic follow up for Basic Referral Services (Exhibit 1) and per case follow up for Enhanced Referral Services (Exhibit 2). The AAA will provide statistical reporting to the FSN on a quarterly basis to be used in the compilation of semiannual reports. Subsequent yearly contracts will be renegotiated based on the statistical reports of units of service previously used. 9) INSURANCE: The AAA agrees to obtain and maintain comprehensive general and professional liability insurance to cover all of its operations and activities including those covered by this agreement. 10) TERMINATION: This Agreement may be terminated by either party upon thirty (30) days written notice to the other. AAA will inform the FSN who the designated counselor is and if AAA changes the designee(s) , AAA will notify the FSN in a timely manner. Linda E. Piper Director 7- /5i- 9_ Date Page 3 of 7 951454 EXHIBIT 1 SCOPE OF SERVICE BASIC ELDER CARE AAA SERVICES PROVIDED: A. Telephone consultation with an Aging Specialist. B. Referral numbers and contacts for out of state services for older adults. C. Referral lists of agencies and phone numbers for local services for older adults. D. Articles for company newsletters on topics of interest to caregivers and their families. E. On -site overview presentations on services available to Weld County older adults and their families. II. AAA RESPONSE OBLIGATIONS: A. "Employee" calls for referral services (A, B, C above) will be addressed immediately if an Aging Specialist is available or within 48 hours of the initial phone request. B. "Employee" requests for overview presentations and/or newsletter material will be referred to the Area Agency on Aging Director and will be responded to within 48 hours of the initial phone request. III. AAA QUALITY ASSURANCE OBLIGATIONS: A. The AAA will maintain current and accurate referral and basic eligibility information. B. The AAA will make available to the "employee" a variety of service alternatives, both public and private, to assist the employee in selecting an appropriate service provider. C. AAA will mail out Quality Assurance questionnaires to clients utilizing contracted resource and referral services. The FSN will receive the original surveys from clients, compile information and report to AAA a semiannual basis regarding these Quality Assurance measures. IV. AAA RECORD KEEPING OBLIGATIONS: A. Statistical reporting for semi-annual reports made for contracting businesses by the FSN. B. Client/employee confidentiality is of primary concern. Statistical information will be used only for reporting purposes. Page 4 of 7 951454 V. AAA DATABASE INFORMATION ON ELDER CARE SERVICES: A. The AAA will maintain a resource file that includes all providers of elder care services in its designated geographic area. This resource list shall include both public and private agencies, programs and services. B. The following minimum information shall be kept on each elder care service provider and updated at least quarterly: 1. Provider's name, address, telephone number, and contact person. 2. Whether licensed, bonded, registered, or exempt from regulation. 3. Days/hours of operation. 4. Holiday schedules (if applicable) 5. Transportation provided/accessibility to public transportation. 6. Information about fees, subsidies, or other financial assistance. 7. Eligibility Information. 8. Application procedure and/or current waiting list. 9. Policies regarding acceptance of persons with Alzheimer's Disease (if applicable) . 10. Brief program description, including any auxiliary service or special facilities. VI. MATERIALS AND SUPPORT PROVIDED BY FSN: A. Confirmation Letter Forms will be provided on which to record the information that is being sent to the caller. Referral Case Record Forms are provided to be used for reporting case data and will be kept on file and accessible to the FSN. Bitting Records will be provided for reimbursement of fees by the FSN. Page 5 of 7 951454 EXHIBIT 2 SCOPE OF SERVICE ENHANCED ELDER CARE AREA AGENCY ON AGING SERVICES PROVIDED: Enhanced resource and referral includes all basic services plus the following: A. One hour face to face elder care consultation with a qualified care manager at the Area Agency on Aging office: 1) One hour consultation will include: identification of care giving issues, development of -a preliminary care plan, service referral alternatives, and a comprehensive packet of written resource information, and a follow up telephone contact with the employee within fourteen (14) working days. OR, 2) Same as above, with consultation to be held at work site B. Participation in employee orientation meetings to explain services available to the "employees" under the terms of the FSN agreement. Cost: $10.00 per hour C. On -site monthly dependent care consultation - Cost: $10.00 per hr. D. Brown Bag Information Sessions - 45 min. to 1 hr. plus prep time Cost: $40.00 E. Other care giving assistance and/or activities can be negotiated with employer. Additional services may be contracted for at an additional expense. An addendum will be added to this contract outlining the service and cost after identification of need and negotiation with service provider. II. AAA RESPONSE OBLIGATIONS: Enhanced response obligations include all basic services plus the following: A. "Employee" calls for face to face consultation services (A above) will be addressed immediately if a Care Manager is available or within 48 hours of the initial phone request. B. Any additional "Employer" requests (C through G) not specified in the initial contract will be coordinated through the FSN with the Area Agency on Aging Director and will be responded to within 48 hours of the initial phone request. An additional fee is charged for services C -G. Page 6 of 7 951454 III. AAA FOLLOW-UP FOR ENHANCED REFERRAL SERVICES: A. The AAA will maintain current and accurate referral and basic eligibility information. B. The AAA will make available to the "employee" a variety of service alternatives, both public and private, to assist the employee in selecting an appropriate service provider. C. AAA will mail out a Quality Assurance questionnaire two to four weeks following the close out date for each unit of service. The FSN will receive the original questionnaires from clients, compile information and report to AAA on a semiannual basis regarding these Quality Assurance measures. IV. AAA RECORD KEEPING OBLIGATIONS: A. The AAA will provide statistical information and maintain confidentiality as specified in the Basic Scope of Services. V. AAA DATABASE INFORMATION ON ELDER CARE SERVICES A. The AAA is expected to maintain a resource file as specified in the Basic Scope of Services. VI. MATERIALS AND SUPPORT PROVIDED BY THE FSN: A. Confirmation Letter Forms will be provided on which to record the information that is being sent to the caller. This information will be kept on file and accessible to the FSN. B. Referral Case Record Forms are provided to be used for reporting case data and will be kept on file and accessible to the FSN. C. Speckman, Executive Director Human Seri}cLcs,' From Family Support Network Financial Agreement Subject: Enclosed for Board approval is a financial agreement with the Family Support Network, a program facilitated by United Way of Weld County. The Family Support Network was established in 1993 to serve as a link between parents, dependent care providers, employers, and the community. Through this agreement the Weld County Area Agency on Aging will be reimbursed for services ranging from information and referral, educational presentations on eldercare, and one on one case management for Weld County employers who request dependent care services through the Family Support Network. Reimbursement rates will vary depending on the type of service. The amount of activity will depend on employer requests for dependent care assistance.
