RE: APPROVE CONTRACT RENEWAL LETTER#1 FOR TB OUTREACH AND CONTROL
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter#1 for the TB
Outreach and Control Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
January 1, 1999, arid ending June 30, 1999, with further terms and conditions being as stated
in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract Renewal Letter#1 for the TB Outreach and Control
Contract between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 13th day of January, A.D., 1999, nunc pro tunc January 1, 1999.

Renewal Letter#1 to TB Outreach and C ntrol Contract
Subject:
Enclosed for Board review and approval is Renewal Letter#1 to the TB Outreach and Control
Contract between Weld County Health Department (WCHD) and the Colorado Department of
Public Health and Environment.
Under the provisions of the contract, WCHD will provide active TB treatment, TB preventive
treatment, provide :For and interpret chest x-rays and other medical evaluation services, conduct
contact investigations and TB skin test screening programs. WCHD will also follow-up with
active patients to ensure they receive appropriate information and education and that they adhere
to the completion of their TB therapy.
For these services, the Health Department will receive an amount not to exceed $22,379 for the
period January 1, 1999 through June 30, 1999.

Contract Renewal Letter No. 1
Pursuant to paragraph 1 of"Part C" of the contract with routing number 98 FAA 08149 and contract encumbrance
number EPI9808149, (as amended by Amendment#1, routing number 99 FAA 00139), hereinafter referred to as the
"Original Contract" (a copy of which is attached hereto and by this reference made a part hereof) between the State of
Colorado,Department of Public Health and Environment and Weld County Health Department , for the renewal
term from January 1. 1999 through June 30. 1999,the parties agree that the maximum amount payable by the State for
the eligible services refe;:enced in paragraph 1 of"Part C" of the Original Contract is increased by TWENTY TWO
THOUSAND THREE HUNDRED SEVENY NINE DOLLARS ($22,379.00) to a new total of SEVENTY NINE
THOUSAND FIVE HUNDRED SEVENTY DOLLARS (579,570.001, according to the revised budget below. The
first sentence in paragraph 1 of"Part C"of the Original Contract is hereby modified accordingly. All other terms and
conditions of the Original Contract are hereby reaffirmed.
