RE: APPROVE CONTRACT RENEWAL #1 FOR CANCER PREVENTION PROGRAM
BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal #1 for the Cancer
Prevention Program between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, commencing August 1, 1995, and ending
July 15, 1996, with further terms and conditions being as stated in said contract renewal, and
WHEREAS, after review, the Board deems it advisable to approve said contract renewal,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Contract Renewal #1 for the Cancer Prevention Program between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, and the Colorado Department of Public
Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said contract renewal.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 23rd day of August, A.D., 1995, nunc pro tunc August 1, 1995.

According to Paragraph 10 of Contract PPG950782, Routing Number 950782, between the State of
Colorado, Department of Public Health and Environment, Prevention Division, Cancer Prevention and
Control Program aid Weld County Health Department, the maximum amount of Two Thousand Seven
Hundred and Thirteen Dollars ($2,713.00) is hereby allocated for the next fiscal year beginning August 1,
1995 and ending July 15, 1996.
This amount was determined using the 1995 Medicare reimbursement rates for authorized services as
shown and the projected number of services to be provided as shown In Exhibit A. Paragraph 10 is hereby
modified accordingly. The terms and conditions of the original contract shall remain the same, except for
the addition of the case manager dollars which terms and conditions to the contract are displayed in
Exhibit B which Is hereby incorporated by reference.
This letter of amendment to the contract is intended to be effective August 1, 1995, but in no event shall
this amendment be deemed valid until it shall have been approved by the State Controller or such assistant
as he may designate,

Exhibit B
Funding for a Case Manager is being provided in the annual budget (Exhibit A). This funding Is
to facilitate case enrollment, data completion, and data submission to the CPCP. This funding is based on
650 per case based on the number of budgeted screening cases and consistent adherence to the following
standards of performance. Failure to accept funding for the Case Manager does not exempt the
Contractor from meeting these compliance standards.
Case enrollment forms must be submitted to the program for 90% or more of the cases
within 45 days of the dates of service.
Case enrollment forms must be submitted with essential information properly completed in
90% or more of the cases.
Mammogram results must be submitted to the CPCP in 90% or more of the cases within
45 days of the service being provided. This information must be submitted on the CPCP
designated form or approved dictated reports from the mammography center which include all
required data elements.
Mammogram results must have the essential information reported properly and accurately
in 90% or more of the cases submitted.
Cases involving screening breast or cervical abnormalities must have diagnostic follow-up
Information provided to the CPCP in 90% or more of the cases within 60 days from the date of
detection of the abnormality. This includes women who are referred to outside health care
providers for diagnostic procedures.
Breast and Cervical Diagnostic Follow-up Reports must be submitted with essential
information properly completed in 90% or more of the cases.
Women with screening abnormalities will be required to have either completed diagnostic
follow-up or have documentation of lost to follow-up submitted to the CPCP on every case (100%).
A copy of the documented follow-up efforts must be submitted to the CPCP with the appropriate
diagnostic follow-up form. The minimum follow-up effort accepted will be one registered/certified
letter, but could include telephone and regular mall contacts.
Planned diagnostic follow-up procedures must be completed and submitted to the CPCP in
90% of the cases within 45 days of the planned date. The completed procedure does not have to be
the same as the one planned, but follow-up must be completed in the recommended timeframe to
allow for continuity of care.
Women with cervical abnormalities of CIN II or more severe, or a breast malignancy must
have completed or initiated treatment information submitted to the program in 90% of the cases
within 45 days following the date of diagnosis.
Women with cervical abnormalities of CIN II or more severe, or a breast malignancy will
be considered lost to follow-up in 10% or less of cases. A copy of the documented follow-up efforts
must be submitted to the CPCP with the appropriate diagnostic follow-up/treatment form. The
minimum follow-up effort accepted will be one registered letter, but could Include telephone and
regular mall contacts which will be documented and submitted to the CPCP on the appropriate
form.
The CPCP computer generated breast and cervical follow-up forms must be accurately
completed and submitted for 90% or more of the cases within 45 days of the date of form
production.
The funding for the Case Manager will be paid per individual case based on the number of cases
submitted to the program and continued compliance with the above standards. Adjustments will be any of
the above standards on which there has been non-compliance and will require the Contractor to bring that
standard into compliance within thirty days. More than two breaches of compliance regardless of
remediation, or any one without the proper remediation within thirty days during the term of this
amendment will result in cessation of funding for the Case Manger. A monthly report will be provided to
the Contractor showing the ratio of actual cases to budgeted cases and the compliance statistics to the
above standards.

Renewal Letter for Cancer Prevention Program Contract
Enclosed for Board approval is Renewal Letter #1 to the Cancer Prevention Program Contract for
women under the age of 40. This contract is between Weld County Health Department and the
Colorado Department of Public Health and Environment.
The term of this renewal is from August 1, 1995 through July 15, 1996. It provides for a
maximum reimbursement of $2,713 which is a decrease of $3,399 from the previous contract.
This renewal deletes the provision for new screenings for women of this age group. These
services were previously funded at $3,092. It also decreases the number of clients served from 15
to 10 for colposcopy services, and it adds funding for case management.
I recommend your approval of this renewal letter. However, with the decrease in funding, we
may consider not renewing the contract in the future.
