RE: APPROVE CHANGE ORDER LETTER NO. 1 BETWEEN AREA AGENCY ON AGING
AND COLORADO DEPARTMENT OF HEALTH CARE POLICY AND FINANCING
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter No. 1 between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and the
Colorado Department of Health Care Policy and Financing, commencing July 1, 1996, and
ending June 30, 1997, with further terms and conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter No. 1 between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the Colorado Department of
Health Care Policy and Financing be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of April, A.D., 1997, nunc pro tunc July 1, 1996.

In accordance with Paragraph 7 of Page 2 of Exhibit A of contract routing number 3397-1324 between the
State of Colorado Department of Health Care Policy and Financing and Weld County Board of
Commissioners by and through the Weld County Division of Human Services Area Agency on Aging,
1551 North 17th Avenue, Greeley, CO 80631 covering the period of July I, 1996 through June 30, 1997
the undersigned agree the first sentence of Paragraph 9, Page 7, Compensation/Maximum Payable is
hereby amended to read:
Payment pursuant to this Contract will be made as earned, in whole or in part, from
available state funds encumbered in an amount not to exceed $266,115 for the purpose of
the within described services.
This constitutes an decrease in the maximum compensation to be made to the Contractor by the State under
this contract of $6.280.
The sentences on Page 2 of Exhibit A to the contract which now read:
However, total compensation for the contract term shall not be increased under this
mechanism by more than $32,970 above the base contract amount, i.e., to a maximum
total compensation of $272,395. The maximum increase above the base contract amount
represents 42 clients above the base client figure; the figure of 347 clients shall be known
as the "maximum reimbursable client figure".

Attached is a revision to the July 1996- June 1997 between the State of Colorado Department of
Health Care Policy and Financing and Weld County Board of Commissioners by and through the
Weld County Division of Human Services' Area Agency on Aging.
The total grant amount will be reduced from $272,395 to $266,115 due to the average number of
clients served in the 3rd quarter of this fiscal year.
