Home My WebLink About960094.tiff RESOLUTION RE: APPROVE SIX PURCHASE OF SERVICES AGREEMENT AMENDMENTS BETWEEN HUMAN SERVICES AREA AGENCY ON AGING AND VARIOUS ENTITIES AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with six Purchase of Services Agreement Amendments between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of Weld County Human Services Area Agency on Aging, and the following entities: 1. Meals on Wheels, Inc. 2. Catholic Charities Northern 3. Eldergarden Adult Day Program 4. RVNA Home Health Care, Inc. 5. North Colorado Medical Center Foundation, Inc. 6. Charles J. Connell, Attorney at Law WHEREAS, said amendments commence January 2, 1996, and end December 31, 1996, with further terms and conditions being as stated in said amendments, and WHEREAS, after review, the Board deems it advisable to approve said amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the six Purchase of Services Agreement Amendments between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of Weld County Human Services Area Agency on Aging, and the above named entities be, and hereby are, approved. WITNESSETH: WHEREAS, the "AAA" desires to continue to purchase home delivered meals from " MOW", and WHEREAS, the "AAA" and " MOW" are desirous of amending the original agreement of January 2, 1995, NOW THEREFORE, it is mutually agreed: 1. The "AAA" will reimburse " MOW" $26,885 for 24,650 home delivered meals for 100 persons, at least 20 to be minority. 2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. 3. The terms of this Amendment extend from January 2, 1996 through December 31, 1996. This Amendment will be terminated immediately in the event of loss of funding. (a non profit agency of the Weld Mental Health Center) for $22,550 to / provide 1,473 contact units of peer counseling to 90 elders. t✓ .Purchase of Services Agreement with North Colorado Medical Center Foundation Seniors are Special Program for $9,500 for 1000 units of in-home respite care for 15 elders and their families. 1.7 Purchase of Services Agreement with Charles J.
