Staff of the Greeley Hospital , such qualified practitioners as indicate their desire to become members of the staff by subscribing to the Rules and Regulations . Said staff rules and regulations being as follows , to-wit : STAFF RULES AN]) REGULATIONS In order to establish and maintain a. high standard of scientific work and to conform to the requirements of the Committee of hospital Standardization and Classification, the physicians and surgeons practicing in the Greeley Hospital in Field County , Colorado , have organized a staff consisting of active and associate members . Active .members will subscribe to all the rules and. regulations detailed below . Associate members will subscribe to all the rules and re.riulations except the one pertaining to attendance at monthly staff meetings . ( 1 ) `lhe Greeley Hospital shall keep in a systematic manner case records of all patients treated in the Hospital , together with convenient summary of each case ; �-nd these records shall be utilized in analyses to ascertain the efficiency of the medical and surgical work done in the Hospital . ( 2 ) The case records of the Hospital s , all contain the following data for each patient treated : Identification of the patient by name or number ; name of physician or surgeon responsible for the case ; personal history of patient relevant to complaint ; diagnosis on which treatment is based ; laboratory and physical findings ; important points o.f.operation or of treatment ; post-operative diagnosis ; complications of convalescence ; following up or progess notes ; and , in case of death , autopsy findings when available . ( 3 ) It shall be the duty of tie historian to record the necessary data on forms agreed upon by the Staff . ( 4 ) The date may be recorded by the attending physician or surgeon, or by an interne or by the historian , but failure on the part of the attending physician or surgeon to furnish the record and to give the necessary -lid to the historian in the recording data shall automatically terminate his membership on the Staff . ( 5 ) It shall be the duty of the historian to make out a summary card for each patient , together with the follow-up record . It shall be her duty to make statistical compilations of the results obtained for analysis and review by the Staff of the Hospital . In non-emergency operative cases the history and record of the nhysical examination shall be delivered to the Hospit _: 1 by the surgeon, or ample time shall be allowed for taking the history of surgical patients after patient ruches the Hospital . (8 ) In n.11 surgical cases the surgeon shall , previous to operation , record the diagnosis on which his treatment is based . This diagnosis shall be posted on the board where operatic .ns are listed. ( 9 ) During or at the close of each operp.tion , a description of the pathological findings and of the operation shall be recorded , together with a diagnosis based on the gross pathology. This shall be dictated by the surgeon himself . AGREEMENT - VOLLMAR- FT-LUPTON ROAD./6" On motion the following agreement was made and entered into by and between the Board of County Commissioners and The Independent Sugar Corporation, to-wit : MEMORANDUM OF AGREEMENT between - The Board of County Commissioners of Weld County , Colorado , and II The Independent Sugar Corporation, Ft . Lupton, Colorado . The Board of County Commissioners agree to have graveled a section of the road between Vollmar and Ft . Lupton which section is approximately described as follows : Beginning at the N . W . corner of the SW of the SW- of Section 18 , Township 2 N . , Range 66 W , thence south 14 miles , thence east to the bridge across the South Platte River, a distance of approximately - miles . The said Board further agrees that the work shall be done in a workmanlike manner , using thereon not less than 1350 cubic yards of gravel of the proper size and mixture . The Independent Sugar Corporation agrees that when the above work is completed, they will pay to the Board of County Commissioners 50% of the cost of said work. Agreement for Mainten.ance.. ., Commissioner F .L .Powars moved the adoption of the following Resolution : WHEREAS , under cooperative agreement between the Board of County Commissioners of Weld County, the Colorado Experimental Station of Fort Collins and the Bureau of Plant Industry of the U . S .Department of Agriculttre , as pursuant to Chapter one of the Session Laes of 1915 , there has been extablished in Weld County , as Experimental Station near Greeley, upon leased land ; and WHEhEAS , Weld County, as its portion of cooperation in said Experiment Station, has been paying the sum of one thousand dollars ( $1000 . 00 ) yearly for rental of the land for said Station; and WHEREAS, it is highly important to continue said Station on a permanent basis and to acquire title to land therefor , NOW, THEREFORE, as an inducement to the State of Colorado to appropriate one half share of the cost of purchase of land for said purpose , BE IT RESOLVED, by the Board of County Commissioners of Weld County, Colorado , that Weld County will make available for the purchase of land for said Experiment Station , a sum of money up to Ten Thousand ( $10 ,000 .00 ) such as will equal an appropriation to be made for such purpose by the 1923 Legislature , so that said Station may be maintained under such agreement and such acts of the Legislature and of the United States Government and the said State Agricultural College and the Bureau of Plant Industry and the Board of County Commissioners of Weld County, as shall be adopted relative thereto" Motion was duly seconded and being put to vote , was carried unanimously .
