HomeMy WebLinkAbout992190.tiff RESOLUTION RE: APPROVE CHANGE ORDER LETTER #1 FOR HIV PREVENTION SERVICES CONTRACT AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Change Order Letter#1 for the HIV Prevention Services Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing January 1, 1999, and ending December 31, 1999, with further terms and conditions being as stated in said change order letter, and WHEREAS, after review, the Board deems it advisable to approve said change order letter, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that Change Order Letter#1 for the HIV Prevention Services Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said change order letter. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 30th day of August, A.D., 1999, nunc pro tunc January 1, 1999. Pickle, Director, Department of Public Health and, Environment (i DATE: August 25, 1999 SUBJECT: Change Order Letter Number One to HIV Prevention Services Contract Enclosed for Board review and approval is Change Order Letter Number One to the HIV Prevention Services Contract between Weld County Department of Public Health and Environment (WCDPHE) and the Colorado Department of Public Health and Environment. The purpose of this change order letter is to provide funding to mobilize communities and to develop an integrated network and programming to implement health education/risk reduction activities and services in Northeastern Colorado. For these services, WCDPHE will receive an additional amount not to exceed $5,330 which will bring the total funding of the contract to $24,690 for the time period January 1, 1999 through December 31, 1999. DIRECTOR Attachment 4 Contract SET9900106 COMMUNITY MOBILIZATION FOR HIV/AIDS PREVENTION Northeastern Colorado 1999 (Primary Counties: Larimer, Weld, Logan and Morgan) (Secondary Counties: Washington,Phillips, Sedgwick and Yuma) PURPOSE: To mobilize community(ies)to develop an integrated network and programming to implement Health Education/Risk Reduction activities and services in Northeastem Colorado. The primary target population is men who have sex with men(MSM)and men who have sex with men who do not identify as gay(MSMNGI), the highest risk population as identified by the CDC,CDPHE,and CWT. PROPOSED ACTIVITIES: There are eight counties in rural northeastern Colorado. The project will concentrate efforts in the more populated primary counties. These"primary counties"are Larimer, Weld,Logan and Morgan. The"secondary counties"are Washington, Phillips, Sedgwick and Yuma. Secondary counties are less populated and are serviced by several identified potential health Education/Risk Reduction providers and agencies. These provide services in secondary counties on a part- time or limited basis. Utilizing these services will create more of a comprehensive HIV network for the region. Activities: I. Convene three initiating committees in Larimer, Weld, and the six counties east of Weld, concentrating in Logan and Morgan to: A. Identify all potential stakeholders. B. Inventory existing HIV prevention resources. C. Identify gaps in services, under-reached populations, and isolated geographical regions. D. Develop a plan to: 1. Increase and improve services from existing providers. 2. Identify interventions to implement to reach high-risk individuals in each region with multi- pronged prevention strategies. QUESTIONS TO BE ANSWERED: • Who is providing prevention services in your area? • Who is conducting information and referral services? • What is the source of funding for these services? Amount? • What services are being provided? To whom? Where? • What are the numbers and characteristics of unnerved or unreached populations? • What prevention messages are being delivered? • How effective is the current prevention network? How can it be improved? Page 1 of 2 TIMELINE: September Meet with major stakeholders to present program and obtain information of existing services,stakeholders and potential providers • Conduct three regional meetings to accomplish item 1. • Plan additional meetings to implement,coordinating with CDPHE and CWT integrating with the RFP process into ongoing service cevelopment for the region. • Bring additional potential providers to the table as identified in the first meeting. • Pilot system integration to identify additional needs. October • Provide information to providers regarding RFP process. • Identify potential additional funders. • Assist in develop ng and reviewing proposals for funding. • Evaluate and review progress,make formative adjustments. • Support futher system development. • Conduct regional planning and goal development. November Support further system development. • Establish community advisory boards for each region. • Access results of CDPHE Application Process and plan additional funding development. December Evaluate program. • Develop plan to implement HIV prevention activities for FY2000. • prepare project report with recommendations. FUNDS& BUDGET The budget is attached(Attachment 5). Funds are to be dispensed by Weld County Health Department through an in-kind service agreed upon by Gerald E.Ernst, and John Pickle, Director of Weld County Health Department.
