RE: APPROVE CHANGE ORDER LETTER#1 FOR DIABETES CONTROL CONTRACT
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Health Department, and the Colorado Department of Public
Health and Environment, commencing July 1, 1997, and ending June 30, 1998, with further
terms and conditions being as stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter#1 between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Health Department, and the Colorado Department of Public Health and Environment be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 11th day of March, A.D., 1998, nunc pro tunc July 1, 1997.

Dear Mr. Pickle,
In accordance with Paragraph 5 of the contract with routing number 9807231 and contract number PPG9807231
hereinafter referred to as the Original Contract(copy attached and by this reference made a part hereof) between the
State of Colorado, Department of Public Health and Environment, EMS & Prevention Division and WELD COUNTY
HEALTH DEPARTMENT, covering the period of July 1 , 1997 through June 30 1998 , the parties agree that the
maximum amount payable by the State for the eligible services in Paragraph 1 of the Original Contract is being
increased by ONE THOUSAND NINE HUNDRED FIFTY DOLLARS ($ t950.00 ) for operating expenses to a new
total of SEVEN THOUSAND THREE HUNDRED TWENTY-FIVE DOLLARS ($ 7,325.00 ). The first sentence in
Paragraph 3 of the Original Contract is hereby modified accordingly. All other terms of conditions of the Original
Contract are hereby reaffirmed.
This amendment to the Original Contract is intended to be effective as of March 1, 1998, but in no event shall it be
deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.

Subject: Change Order Letter for Diabetes Control Contract
Enclosed for Board review and approval is Change Order Letter No 1 between the Weld County
Health Department and Colorado Department of Public Health and Environment for the Diabetes
Control Program Contract.
This change order will increase the amount payable by the State to Weld County by $1,950
from a maximum amount of$5.375 to $7,325 for the contract period July 1, 1997 through June
30, 1998. This funding provides for activities such as coordination of a community-based
diabetes coalition, identification of needs in the community for prevention and dissemination of
this information, and maintenance of English and Spanish books pertaining to diabetes in local
I ibraries.
I recommend your approval of this change order letter.
