RE: APPROVE CONTRACT RENEWAL LETTER#1 FOR STATE PER CAPITA
CONTRACT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Renewal Letter#1 for the
State Per Capita Contract between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
July 1, 1998, and ending June 30, 1999, with further terms and conditions being as stated in said
renewal letter#1, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter#1,
a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Renewal Letter#1 for the State Per Capita Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter#1.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of August, A.D., 1998, nunc pro tunc July 1, 1998.

Contract Renewal Letter No. 1
In accordance with Paragraph 7 of the contract with routing number 9807021 and contract number ADM9807021
hereinafter referred to as the Original Contract(copy attached and by this reference made a part hereof)between the
State of Colorado,Department of Public Health and Environment(Consumer Protection Division)and Weld County
Health Department,the parties hereby agree the Original Contract is hereby renewed for the period of July 1, 1998
through June 30, 1999. The parties agree that the maximum amount payable by the State for the eligible services during
this renewal period is ONE HUNDRED NINETY-ONE THOUSAND ONE HUNDRED FORTY-TWO DOLLARS
($191,142.00),according to the budget attached hereto as Attachment A,incorporated herein. The terms and conditions
of the Original Contract are hereby reaffirmed and shall continue in full force and effect throughout this renewal period
unless otherwise stated.
Paragraph 7,Item A. -Shall be changed to read"Payment equal to three fourths(3/4)of the total allocation
may be advanced to contractor when this contract is fully executed in accordance with the State Fisal Rules and
in subsequent years when the contract renewal letter has been signed by all parties including the State
Controller or his authorized designee. Payment of the final one fourth(1/4)of the total allocation shall be
conditioned upon receipt by the State of reports on forms and in the detail prescribed herein and within the time
schedule as set forth in Paragraph 5 of the original contract.
Paragaph 7,Item B. -Shall be deleted.
This amendment to the Original Contract is intended to be effective as of July 1, 1998, but in no event shall it be
deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.

Enclosed for Board review and approval is the State Per Capita Contract Renewal Letter No. 1
between the Colorado Department of Public Health and Environment and Weld County Health
Department.
Weld County Health Department's appropriation for the State fiscal year July 1, 1998 through
June 30, 1999 is $191,142 which is $9,792 more than our funding for last year. Of this amount,
$28,671 must be used for Regional Services.
I recommend your approval of this renewal letter.
