RE: APPROVE AMENDMENT TO THE CHILD PROTECTION AGREEMENT FOR
SERVICES WITH THE ISLAND GROVE REGIONAL TREATMENT CENTER, INC.,
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and the Island Grove Regional Treatment Center, Inc., commencing March 1, 1997,
and ending May 31, 1997, with further terms and conditions being as stated in said amendment,
and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex -officio Board of Social Services, that the Amendment to the Child
Protection Agreement for Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and the Island Grove Regional Treatment Center, Inc., be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 5th day of March, A.D., 1997, nunc pro tunc March 1, 1997.

AMENDMENT TO THE CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND THE ISLAND GROVE REGIONAL TREATMENT CENTER, INC.
This Amendment, made and entered into the 5 th day of March, 1997, by and between the
Board of Weld County Commissioners, on behalf of the Weld County Department of Social Services,
hereinafter referred to as "Social Services," and the Island Grove Regional Treatment Center, Inc.,
hereinafter referred to as "Island Grove."
WITNESSETH
WHEREAS, required approval, clearance, and coordination have been accomplished from and with
appropriate agencies; and
WHEREAS, Social Services and Island Grove desire to enter into an amendment to amend the
payment schedule and to increase the total reimbursement amount of the Agreement.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as
follows:
1. Term
This Amendment shall become effective March 1, 1997, upon proper execution of this Agreement.
2. Exhibit B, Payment Schedule, Item I. Funding and Method of Payment
Social Services agrees to reimburse to Island Grove in consideration for the work and services
performed, a total amount not to exceed Fifty-two Thousand Three Hundred Twenty-three Dollars
($52,323) under Fund Code 1889 and Object Code 104. This Amendment reflects an allocation
increase of Eight Thousand Six Hundred Twenty-three Dollars ($8,623) from the Family Issues
Cash Fund for the balance of FY96-97.
3. Exhibit B, Payment Schedule, Item 2. Fees for Services
Social Services agrees to pay Island Grove for the costs of services as follows:
a. Assessments
1) Alcohol and Drug Differential Assessment $ 135.00
(Included baseline Urinalysis Test)
2) Domestic Violence $ 135.00

b. Treatment Options
3) Intensive Family Therapy $ 90.00/session
Co-Pay/Sliding Fee

(For pregnant women and postpartum women who are Medicaid reimbursement
eligible.)
• If clients are not Medicaid reimbursement eligible, fees are as stated below:
a) Differential Assessment $ 135.00
2) Fast Tract Adolescent Alcohol/Drug Assessment
a) Alcohol/Drug Differential Assessment $ 135.00
Except as modified herein, the terms and conditions of the original Agreement (No. FY96-3003) together
with all exhibit of the original Agreement, remain in full force and effect.

RE: Amendment to the Child Protection Agreement for Services
between the Weld County Department of Social Services and the
Island Grove Regional Treatment Center, Inc.
Enclosed for Board approval is an Amendment to the Child Protection Agreement for Services
between the Weld County Department of Social Services and the Island Grove Regional Treatment
Center, Inc. The term of the original Agreement remains the same and will end May 31, 1997.
The major provisions of the Amendment are as follows:
1. The effective date of the Amendment is March 1, 1997.
2. The Amendment reflects an allocation increase of $8,623, which increase the total
reimbursement level of the Agreement to $52,323.
3. Fees for services have been adjusted to reflect increased costs to Island Grove Regional
Treatment Center in assessments. Intensive Family Therapy treatment has been added as a
service.
