RE: APPROVE CONTRACT RENEWAL LETTER NO. 2 FOR TUBERCULOSIS OUTREACH
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter No. 2 for
Tuberculosis Outreach Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, Disease Control and
Environmental Epidemiology Division, commencing February 1, 1996, and ending January 31,
1997, with further terms and conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Contract Renewal Letter No. 2 for Tuberculosis Outreach Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department of
Public Health and Environment, Disease Control and Environmental Epidemiology Division, be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 14th day of February, A.D., 1996, nunc pro tunc February 1, 1996.

Contract Renewal Letter No. 2
In accordance with Paragraph 3 of contract routing number 94-0889 of contract number EPI940889,
(copy attached and by this reference made a part hereof) between the State of Colorado Department of
Health (Disease Control and Environmental Epidemiology Division) and Weld County Health
Department covering the period of February 1, 1996 through January 31, 1997 the undersigned agree
that the maximum amount payable by the State for eligible services is $20,899. Funding is contingent
upon continued availability of federal funds for payment pursuant to the terms of this agreement. The
terms and conditions of the original contract shall remain the same.
This amendment to the contract is intended to be effective as of 02/01/96, but in no event shall it be
deemed valid until it shall have been approved by the State Controller or such assistant as he may
designate.

Subject: Renewal Letter for TB Outreach Contract
Enclosed for Board review is a renewal letter between the Weld County Health Department and
the Colorado Department of Public Health and Environment for the Tuberculosis Outreach
Contract.
The amount of funding for the time period February 1, 996 through January 31, 1997 is $20,899
which is the same level of funding as last year.
This contract is necessary to help fund TB follow-up within the County as we continue to follow
several active cases per year and contact investigations for those persons exposed but not
requiring treatment. I recommend your approval of this renewal letter.
