HomeMy WebLinkAbout951103.tiffRESOLUTION RE: APPROVE NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR PLACEMENT ALTERNATIVES COMMISSION (PAC) FUNDS FOR ALTERNATIVE HOMES FOR YOUTH AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Notification of Financial Assistance Award for Placement Alternatives Commission (PAC) Funds between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Alternative Homes for Youth, commencing April 1, 1995, and ending May 31, 1995, with further terms and conditions being as stated in said award, and WHEREAS, after review, the Board deems it advisable to approve said award, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex -officio Board of Social Services, that the Notification of Financial Assistance Award for Placement Alternatives Commission (PAC) Funds between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Alternative Homes for Youth be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said award. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 31st day of May, A.D., 1995, nunc pro tunc April 1, 1995. Enclosures: Signed RFP Monthly Program Capacity 4 Monthly Average Capacity 3 Description The issuance of the Notification of Financial Assistance Award is based upon your Request for Proposal (RFP). The RFP specifies the scope of services and conditions of award. Except where it is in conflict with this NFAA in which case the NFAA governs, the RFP upon which this award is based is an integral part of the action. Special conditions 1) Reimbursement for the Unit of Service will be based on an hourly rate per child or per family. 2) The hourly rate will be paid for only direct face to face contact with the child and/or family or as specified in the unit of cost computation. 3) Unit of service costs cannot exceed the hourly, monthly, and yearly cost per child and/or family. 4) Rates will only be paid on approved and open cases with the Department of Social Services. Based on the Placement Alternatives Commission recommendations, the purpose of these revisions are to: 1. Decrease the yearly budget of Weld Mental Health Center Option A Program from $72,562.00 to $62,562.00. The difference will be $10,000.00 2. Reinvest the amount of $10,000.00 and increase the Weld Mental Health Center Option B Program budget from $22,488.76 to $32,488.76 3. Adjust the hourly rate of Alternative Homes for Youth, Day Treatment Program from $6.08 per hour to $18.00 per hour. This transfer of funds will be effective April 1, 1995.
