RE: APPROVE CONTRACT RENEWAL LETTER #1 TO PERSONAL BOARDING HOME
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter#1 to the
Personal Boarding Home Contract between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment, commencing July 1, 1999, and ending June 30, 2000, with further terms and
conditions being as stated in said contract renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said contract renewal
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Renewal Letter#1 to the Personal Boarding Home
Contract between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 23rd day of June, A.D., 1999.

Dear Mr. Pickle:
Pursuant to paragraph C.8 of the contract with contract routing number 99-00045 and contract encumbrance number
FAA HFD9900045 hereinafter referred to as the"Original Contract"(a copy of which is attached hereto and by
this reference incorporated herein and made a part hereof)between the State of Colorado,Department of Public
Health and Environment and Weld County Department of Public Health and Environment, for the renewal
term from July 1, 1999 through June 30,2000,the parties agree that the maximum amount payable by the State for
the eligible services referenced in paragraph C2 of the Original Contract is increased by SEVEN THOUSAND
EIGHT HUNDRED TWENTY DOLLARS ($7,820)to a new total of EIGHTEEN THOUSAND FIVE
HUNDRED FIFTY DOLLARS. ($18,550). The revised facility list which is attached hereto as Attachment 3 is
made a part hereof and incorporated herein by this reference. Reimbursement shall be made at the flat rate of
THREE HUNDRED FORTY DOLLARS ($340.00)per facility survey. The first and fourth sentence in
paragraph C.2 of the Original Contract is hereby modified accordingly. All other terms and conditions of the
Original Contract are hereby reaffirmed.
This amendment to the Original Contract is intended to be effective as of July 1, 1999. However, in no event shall
this amendment be deemed valid until it shall have been approved by the State Controller or such assistant as he
may designate.
Please sign,date,and return all 4 originals of this Contract Renewal Letter by June 25, 1999 to the attention of:

DATE: Renewal Letter Number One to Personal Care Boarding Home
Contract
Enclosed for Board review and approval is Renewal Letter Number One to the Personal Care
Boarding Home Contract between Weld County Department of Public Health and Environment
(WCDPHE) and the Colorado Department of Public Health and Environment.
Under the provisions of this renewal letter, Weld County will be responsible for annual
inspections of up to 23 personal care boarding home facilities in Weld and Larimer counties and
for investigating complaints in both counties. For these services, the Health Department will be
reimbursed at the rate of$340 per facility surveyed not to exceed $7,820 for the time period
July 1, 1999 through June 30, 2000.
This is a valuable service for a critical public health population, the elderly and mentally ill, who
cannot care for themselves. I recommend your approval of this renewal letter.
