RE: APPROVE LETTER OF INTENT FOR PUBLIC HEALTH CONFERENCE SUPPORT
GRANT PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Letter of Intent for the Public Health
Conference Support Grant Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Health Department, and the Center for Disease Control and Prevention, commencing August 1,
1998, and ending June 30, 1999, with further terms and conditions being as stated in said letter
of intent, and
WHEREAS, after review, the Board deems it advisable to approve said letter of intent, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Letter of Intent for the Public Health Conference Support Grant
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Center for Disease Control and Prevention be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said letter of intent.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 1st day of April, A.D., 1998.

Please accept this letter of intent for consideration of DHHS;CDC&P Announcement#803: Public Health Conference
Support Grant Program.
Title: Regional Health Education Standards Skill Building Workshops
Workshop #1 - "Health Education Program Evaluation" (Video Conference with 5 sites)
DATE: October, 1998. •
Workshop#2-" Strategic Planning for Health Education-Moving from Theory to Action" (Regional Workshops)
DATES: November 1998-March 1999
Location(s): Five regional locations per workshop(10 total): 1) Northwest Colorado/Denver Metro Area; 2) Southeast
Colorado; 3)Southcentral Colorado/Colorado Springs 4)Northwest/West Central Colorado; 5) Southwest Colorado. All 63
counties in Colorado are incorporated into these 5 regions.
Purpose: 1) To provide quality trainings consistent with needs identified and prioritized from statewide assessment of local
public health professionals, including health promotion,nursing, and environmental health specialist; 2) To train individuals in
competencies linked to population based services: 3) To increase skill levels of statewide professionals on topics applicable to:
environmental health; chronic and infectious disease prevention including risk factors of nutrition,tobacco, and physical activity;
and injury control; 4) To facilitate linkages between statewide multi disciplinary public health professionals thereby
strengthening Colorado's public health system; 5) To further national training goals and plans supported by agencies such as
CDC and Robert Wood Johnson. This project addresses Healthy People 2000 Objective 8.1: increase years of healthy life to at
least 65 years.
Intended Audience: These programs are intended for everyone in the public health arena in Colorado including Health Officers
and staff at 13 local public health departments, local health(county nursing)agencies and environmental health services(49)with
emphasis on the rural and underserved areas. Members of the Public Health Association(302),Colorado Environmental Health
Association(300), Connecting Colorado Statewide Coalition(700)and other community agencies will be invited. Expected
audience participation for each workshop is 50,for a total of 500 participants.

SUBJECT: Letter or Intent for DHHS, CDC&P
Public Health Conference Support Grant
Enclosed for board review and approval is a letter of intent for a grant proposal between
Weld County Health Department and the Center for Disease Control and Prevention to
provide health education skill building workshops to local county health department staff,
health officers, public health nurses and environmentalists in all 63 counties in Colorado.
This is a request for supplemental funds to support the Health Education Standards
Preventive Health Block Grant Project. This will enable us to provide training to a greater
number of people through video conference and live presentations in five different
regional locations in Colorado. The staff at all local public health agencies will serve the
high risk population in their communities with improved skills to meet their needs.
The request is for supplemental funds for the funding period August 1, 1998 - June 30,
1999. We are requesting $18,720.00 to maintain this project.
