RE: APPROVE MONITORING AGREEMENT AND AUTHORIZE CHAIR TO SIGN -
SECURITY ALARM SYSTEMS, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Monitoring Agreement between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
Security Alarm Systems, Inc., commencing May 1, 1999, and ending April 30, 2002, with further
terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Monitoring Agreement between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and Security Alarm Systems, Inc.,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 19th day of July, A.D., 1999, nunc pro tunc May 1, 1999.

SUBJECT: Monitoring Agreement for Alarm System
Enclosed for Board review and approval is an agreement between Weld County Department of
Public Health and Environment (WCDPHE) and Security Alarms Systems, Inc. for the
temperature monitoring system on refrigerators used to store vaccine.
Under the provisions of this agreement, Security Alarms Systems, Inc. will continue to monitor
the temperature of refrigerators located in the Health Department's clinics to ensure the
temperature does not rise above or fall below the range determined to be safe for storage of
vaccines. Upon receipt of an alarm signal, Security Alarm Systems will notify designated Health
Department staff members by telephone. The charge for this service is $20 per month for the
time period May 1, 1999 through April 30, 2002. The agreement will automatically renew for
successive periods of three years unless written notice of cancellation is given by either party.

MONITORING AGREEMENT
AGREEMENT made this_/ day of /iziLj , 19_ �T_ , between Security Alarm Systems. Inc.,
(hereinafter called"Security"),and Lek J-1 (j,-; 11:61, t, fie 4/y
(hereinafter severally and collectively called"Subscriber").
1. Security shall provide alarm monitoring service to the premises located at .45-.5_S7 /L'_ 17 .4✓'c

system owned or leased and maintained by Subscriber.
2. Subscriber ighall pay to Security:
(A) Log Fee (plus sales tax, if applicable) on execution of this Agreement. $ /21t(A-
(B) Monthly service charge(plus sales tax, if applicable), payable quarterly (or annually), in advance on the first day of
each quarter(year)during the term of this Agreement(prorated if first month is less than full month). The monthly
service charge shall become due, and the alarm monitoring service shall commence, on the day of connection of
transmission equipment to the telephone lines of Subscriber. (Service charges paid annually, in advance. receive a

Subscriber shall also pay all municipal, state or federal taxes, fees, or assessments now or hereafter applying to
this service. Subscriber shall also pay the installation and monthly telephone charges applicable to the installation and
connection of the system and such charges shall be payable direct to the telephone company. In addition, Subscriber
agrees to pay a late payment interest charge at the rate of one and one-half (1 1/2) percent per month on any unpaid
balances exceeding 30 days.
3. Except as otherwise provided herein,the initial term of this Agreement shall be three(3)years from the date of commencement
of the alarm monitoring service as described above and shall automatically renew itself for a period of three (3) years and for succes-
sive periods of three (3) years each thereafter under the same terms and conditions unless either Security or Subscriber gives written
notice of cancellation to the other, in the form of certified mail, return receipt requested, at least thirty (30) days prior to the end of the
then current term.
4. Upon receipt of an alarm signal, Security will endeavor to notify by telephone. the appropriate police or fire department or
others as designated in the Responsible Party(RP) List, Surveillance Center Procedure. It is understood that Security is not obligated,
under any circumstances,to send any person to the premises of the Subscriber in response to an alarm signal. Security will use its best
efforts to carry out its duties hereunder promptly, but shall not be responsible for delays or failures to perform by reason of busy
telephone facilities, failure of telephone equipment, or failure of telephone lines due to weather or other conditions or otherwise for any
conditions beyond the control of Security.
5. Subscriber acknowledges that he has read and understands this entire Agreement and that he has received a copy of this
Agreement and that there are no oral or written understandings, except as provided herein, which change or modify any of the terms
of the Agreement.
