RE: APPROVE CONTRACT FOR LONG TERM CARE OMBUDSMAN
ADVOCATE/FRIENDLY VISITOR PROGRAM AND AUTHORIZE CHAIR TO SIGN -
VICTIM ASSISTANCE AND LAW ENFORCEMENT BOARD
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract for the Long Term Care
Ombudsman Advocate/Friendly Visitor Program between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the Victim Assistance and Law
Enforcement Board, commencing January 1, 2000, and ending December 31, 2000, with
further terms and conditions being as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract for the Long Term Care Ombudsman
Advocate/Friendly Visitor Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the Victim Assistance and Law Enforcement
Board be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 13th day of December, A.D., 1999.

SUBJECT: VALE Contract for Approval
Enclosed for Board approval is a contract between the Victims Assistance and Law
Enforcement Board (VALE) and Weld County Area Agency on Aging. This contract
commences January 1, 2000 and terminates on December 31, 2000.
The VALE Board will award the Area Agency on Aging $8,500.00 to support
ombudsman advocacy for residents of personal care boarding homes. Activities under
this contract include regular visitation by the ombudsman to personal care boarding
homes, complaint investigation and training for aides working in these facilities.

CONTRACT
CONTRACT between Victim Assistance and Law Enforcement
Board ( "Donor" ) and Weld County Area Agency on Aging
( "Recipient" ) , p0 Box 1805. Greeley, CO 80632 , for the funding
of Weld County ong Term Care Ombudsman Advocate/Friendly
Visitor Program ("Grant" , "Grant Application" or "Project") , as
described in your Grant Application for the period commencing
on January 1, 2000 .
The parties (Donor and Recipient) in consideration of the
mutual promises contained herein agree as follows :
1 . The Recipient hereby contracts to perform the services
outlined in the Grant Application, as submitted to the VALE
Board and incorporated herein by reference.
2 . The parties acknowledge that the Grant contemplated
herein is made pursuant to Colorado law and the Recipient
specifically agrees to use the Grant for the purposes specified
in C.R.S . , 1973 , S24-4 . 2-105, as amended. In addition, the
Recipient agrees to submit to the Donor quarterly reports,
including a "Grant Report Form" and a "Narrative Report . "
Furthermore, EIGHT (8) COPIES of quarterly reports are due not
more than FIFTEEN (15) DAYS after the end of each successive
three month period of the grant term. Failure to submit
quarterly reports may result in withholding of funds and/or
grant termination.
3 . The Grant commences on January 1, 2000, and terminates
on December 31 , 2000 .
4 . The Donor hereby grants to the Recipient the sum of
58 . 500 . 00 of which an initial sum of 52 , 125 . 00 will be paid to
the Recipient on or about January 15. 2000 . The projected
disbursement of the balance will be paid as follows :
52 , 125 . 00 on or about April 15 , 2000,
52, 125 . 00 on or about July 15 . 2000, and
52 ,125 . 00 on or about October 15, 2000 .

5 . The Grant may be terminated by the Donor if :
(a) the Recipient does not submit the required
quarterly reports, or if the reports or other information does
not show adequate achievement of the Grant purpose (s) as stated
on the Grant Application and pursuant to C.R.S . , 1973 S24-4 . 1-
105, as amended; or,
(b) the Donor has insufficient funds to continue the
Grant .
Before termination for inadequate achievement, the
Recipient shall have an opportunity to meet with the Donor.
The Grant may be terminated by the Recipient at any time .
6 . At the end of the Contract term, or upon Contract
termination, all unspent funds shall be returned to the Donor.
7 . The Contract termination date may be extended by
written mutual agreement of the Donor and Recipient . The
services to be provided may also be modified by written mutual
agreement of both parties (Donor and Recipient) .
The signatories certify that we have read the Contract and are
fully cognizant of our duties and responsibilities for this
Project .
