RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR FAMILY
GROUP DECISION MAKING SERVICES AND AUTHORIZE CHAIR TO SIGN -
ACKERMAN AND ASSOCIATES, P.C.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Family Group Decision Making Services between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Social Services, and Ackerman and Associates, P.C., commencing June 1,
1999, and ending May 31, 2000, with further terms and conditions being as stated in said
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to the Child
Protection Agreement for Family Group Decision Making Services between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Social Services, and Ackerman and Associates, P.C., be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of June, A.D., 1999, nunc pro tunc June 1, 1999.

Between the Weld County Department of Social Services
and Ackerman and Associates, P.C.
Enclosed for Board approval is an amendment to the Child Protection Agreement for
Services between the Weld County Department of Social Services and Ackerman and
Associates, P.C. The amendment is necessary to reflect our notification by the Colorado
Department of Human Services of decreased funding from the Family Issues Cash Fund
(100% Federal and State Funding) of 41.6%
Ackerman and Associates provide Family Group Decision-Making services. The total
reimbursement authorized under the amendment has been decreased from $75,000 to
$41,910.00. The number of families to be served has also been decreased accordingly.
Twenty-two families are scheduled to be served between June 1, 1999 through May 31,
2000.

This Agreement, made and entered into the 21st day of June, 1999, by and between the County of
Weld, Colorado, by and through the Board of County Commissioners of the County of Weld, on behalf of the
Weld County Department of Social Services, and sitting as the Board of Social Services for the Weld County
Department of Social Services, whose address 915 10 Street, Greeley, Colorado, 80631, hereinafter collectively
referred to as the"County"; and Ackerman and Associates, P.C., hereinafter referred to as the "Contractor."
WITNESSETH
WHEREAS, the County entered into an Agreement with the Contractor on June 1, 1999, to assist
Social Services in providing Family Group Decision-Making services for the Expedited Permanency Planning
Program, and
WHEREAS, the parties hereto now desire to amend the Original Agreement so as to better define such
respective rights and responsibilities, with the Amendments being based upon the need to reflect the revised
budget of the Expedited Permanency Planning Program.
NOW THEREFORE, in consideration of the mutual promises and covenants stated herein, the parties
hereto agree to amend the Original Agreement as follows:
Amend Exhibit A, Scope of Services, to read as follows:
1. Scope of Services
(Paragraph 3) The Services by the Contractor will be no more than 22 families at a rate of$1,875.00
per each completed Family Group Decision Making Conference. In the event a Family Group Decision
Making Conference does not take place, the Contractor shall be paid for the hours of service they have
provided at a rate of$75.00 per hour, to a maximum of 15 hours, or a total maximum of$1,125.00.
The total cost for all 22 families shall not exceed$41,250.00. Any amount in excess of these rates must
have prior approval from Social Services. Social Services agrees to pay the Contractor for refreshments
at each conference at a rate of $30.00 per conference for 22 conferences for a total amount not to
exceed $660.00. Payment for refreshments at each conference in excess of$30.00 must be authorized
by Social Services.

Amend Exhibit B, Payment Schedule, to read as follows:
1. Funding and Method of Payment
Social Services agrees to reimburse to the Contractor, in consideration for the work and services
performed, a total amount not to exceed Forty-One Thousand, Two-Hundred-Fifty Dollars
($41,250.00). Social Services agrees to pay the contractor for the cost of refreshments for each Family
Group Conference at a rate of$30.00 for 22 conferences for a total not to exceed Six Hundred Sixty
Dollars ($660.00). The source of funds to be reimbursed to the Contractor is the Expedited
Permanency Planning Program.
2. Fees for Services
The Services by the Contractor will be no more than 40 families at a rate of $1,875.00 per each
completed Family Group Decision Making Conference. In the event a Family Group Decision Making
Conference does not take place, the Contractor shall be paid for the hours of service they have
provided at a rate of$75.00 per hour, to a maximum of 15 hours, or a total maximum of$1,125.00.
The total cost for 22 families shall not exceed $41,250.00. Any amount in excess of these rates must

Except as modified herein, the terms and conditions of the Original Agreement (PY99-CPS-5)
modifications to the Original Agreement (PY99-CPS-5), together with all attachments of the Original Agreement
remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto have duly executed the Agreement as of the day, month,
and year first above itten. ,
