HomeMy WebLinkAbout970137.tiffRESOLUTION RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES WITH WELD MENTAL HEALTH CENTER, INC., AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Amendment to the Child Protection Agreement for Services between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and the Weld Mental Health Center, Inc., commencing January 1, 1997, and ending May 31, 1997, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex -officio Board of Social Services, that the Amendment to Child Protection Agreement for Services between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and the Weld Mental Health Center, Inc., be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said agreement. Kirkmeyer 970137 SS0023 Contract No. FY96-12000 Amendment 2 AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND THE WELD MENTAL HEALTH CENTER, INC. THIS AMENDMENT made and entered into this day of January 1997, by and between the Board of Weld County Commissioners, on behalf of the Weld County Department of Social Services, hereinafter referred to as "Social Services," and the Weld Mental Health Center, Inc., hereinafter referred to as "Mental Health." WITNESSETH WHEREAS, the parties desire to modify their current contract for child protection services (FY96-12000) in order to expand the scope of services; and WHEREAS, the authority exists to disburse funds and in the Law and Family Issues Cash Funds have been budgeted, appropriated and otherwise made available; and WHEREAS, required approval, clearance, and coordination have been accomplished from and with appropriate agencies. NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows: 1. This Amendment shall be effective January 1, 1997. 2. The term of the Amendment shall be extended to May 31, 1997. 3. Exhibit A, Scope of Services, shall be revised as follows: IV. Special Services A. Target/Eligible Populations The population to be served by this proposed plan includes abused and neglected children who are at risk of out -of -home placement, who are clients of the Weld County department of Social Services, not eligible for Medicaid, and in need of mental health services. The family members of these children and adolescents will also be eligible for services. Page 1 of 5 970137 Contract No. FY96-12000 Amendment 2 B. Types of Services to be Provided In addition to the services already described in the existing plan, the following will be provided: 1) Individual Group, Family, and Couples Mental Health Therapy Direct therapy services will be offered to eligible individuals and families. This will consist of services provided in a variety of modalities, utilizing intervention strategies that are within the scope of generally accepted and recognized mental health practice. These services will be provided by qualified mental health professionals, functioning under the regulatory and clinical supervision system of the Weld Mental Health Center. 2) In -Home Coordination and Intervention In line with the emerging standard of "expedited permanency" it is anticipated that there will be a need for intensive in -home clinical and nonclinical support for targeted families. Such services are available through existing PAC -funded programming, but are limited by allocation amounts. Including these services in this proposal would allow for expanded staffing in this area, as referrals identified need. Trained clinical staff would be expected to interact with selected families in a therapeutic and supportive case management role, helping them to organize and discharge many of the functions associated with daily living. This would be a combined clinical and case management position, involving activities as diverse as providing an evaluation of need, performing in -home therapy, coordinating service acquisition, assisting with scheduling, and providing support in agency interactions. 3) Medication Evaluations Evaluation for the appropriateness of psychotropic medication will be performed, according to the availability of psychiatric services. These could constitute one -session evaluations, or be performed in conjunction with ongoing service delivery. C. Service Time Frames Page 2 of 5 970137 Contract No. FY96-12000 Amendment 2 The current standard within WMHC is to service all routine requests for services within ten (10) working days, with five (5) days being the target. All referrals under this plan would be serviced according to that standard, i.e. there would be an intake performed in no more than ten (10) days from time of referral. This would include referrals for in -home services, which are now somewhat more restricted by PAC funding limitations. Emergency referrals will continue to have immediate access to services. Direct therapy services would be designed to meet the specific needs of referred individuals. The intent would be to provide intense initial intervention, and not to continue such service beyond the scope of clinical need. Such decisions would be made in accordance with existing treatment protocols and utilization management procedures of the Weld Mental Health Center. Access to psychiatry for medication evaluations is somewhat more limited, due to resource restrictions. The service goal is to meet a two-week standard, i.e., provide evaluations within two (2) weeks of referral. In emergency situations, there will continue to be immediate access. D. Measurable Outcomes Faster and more timely responses from the Children and family Services Outpatient Program will occur and therapeutic interventions will be better coordinated between agencies. All of these outcomes can be quantified and compared with status prior to implementation of the Department of Social Services and the Mental Health Center jointly assign measures that will service as specific outcome targets, such as: 1) Intervention will be available within ten (10) days of request for routine referrals and immediately for emergency referrals. 2) For involved referrals, there will be a measurable reduction in the need for out -of -home placements. E. Referrals Referrals for services described in the Addendum will be made by the Weld County Department of Social Services. 4. Exhibit B, Payment Schedule, shall be revised as follows: Page 3 of 5 970137 Contract No. FY96-12000 Amendment 2 III. Special Services A. Rate Structure/Service Provider Staff involved in the provision of services will be fully qualified mental health professionals, functioning under the clinical and administrative supervision systems of the Weld Mental Health Center. Services will be documented and billed at a standard and agreed upon unit -cost rate. B. Funding and Method of Payment Addendum services will be totally funded through existing and available Family Issue Cash Funds. Billings to Weld County Department of Social Services will not exceed funds (Fund Code 1888 and Object Code 104) available through May 31, 1997. 5. Except as modified herein, the terms and conditions of the original agreement (FY96-1200) and modifications to the original agreement (FY96-1200), together with all attachments of the original agreement, remain in full force and effect.
