The coalition meets monthly, and its' members are from both the public and private sectors. Members include representatives of Rotary, Kiwanis, School District 6, Salud Family Health Center, the Women, Infants, and Children Program(WIC), Sunrise Community Health Center, the Weld County Medical Society, North Colorado Medical Center, the Weld Board of Cooperative Educational Services (BOCES), Headstart, the Weld County Health Department, and the Weld County Department of Social Services. The purpose of the coalition is to increase the percentage of Weld County children fully immunized by age two. In order to be fully immunized by age two, a child needs to have had four DTP immunizations, three polio immunizations, one MMR immunization, three to four Hib immunizations, and three Hepatitis B immunizations. According to the 1993-1994 Retrospective Kindergarten Immunization Survey, 52.6 °/0 of Weld County children were fully immunized by age two. It is both a local and a national priority to increase this percentage to 90% by the end of 1996. It is especially important that children under two years of age be fully immunized because they are at greatest risk of getting the diseases the immunizations prevent. In addition, they are likely to have serious medical complications from the diseases. The diseases still exist. There were more cases of pertussis (whooping cough) in Colorado during 1994 than at any time since 1984. ORGANIZATION GOALS: (cont.) 1. A. Providing immunization information to media on a monthly basis. B. Providing immunization information to target audiences: a. Health care providers b. Child care providers c. Providing free immunizations for children under two years of age C. Providing immunization information in two languages D. Reducing missed opportunities in health care providers' offices E. Providing children's immunization clinics throughout Weld County 4. To maximize participation of community resources by: A. Maintaining current coalition membership and increasing membership by two organizations each year B. Doing one speaking engagement per quarter to an agency, health care provider, or other community resource 960108 t AMOUNT OF MONEY REQUESTED: (cont.) $1,500.00 to print'30,000 fliers at five cents each for the second quarter(April,.May, June), third quarter (July, August, September), and fourth quarter(October, November, December). DESCRIBE HOW THE MONEY WILL BE USED TO IMPROVE PRESENT PROGRAM(S): (cont.) These two-sided, bilingual, brightly colored fliers advertised immunization clinics offered throughout the year by the Weld County Health department. The fliers included messages to parents about the importance of timely, age-appropriate immunizations. These fliers were purchased with funding by the Greeley Area Foundation. The coalition is requesting Greeley Area Foundation funding to distribute similar fliers to Weld County parents throughout 1996. The 1996 fliers will include a listing of immunization clinics offered by the Weld County Health Department, Salud Family Health Center, and Sunrise Community Health Center. According to a 1994 Centers for Disease Control and Prevention survey, there has been an increase in the percentage of Colorado two-year-olds fully immunized from 61.6% in 1991 to 75% in 1994. (No data is available specifically for Weld County.) However, it is important that the immunization message continue to be advertised to Weld County parents in order to reach a 90% immunization rate by the end of this year. Many parents continue to lack information about the importance of timely immunizations for children under two years of age. A 1991 Gallup Organization, Inc. survey conducted for the American Academy of Pediatrics determined the extent to which adults responsible for the care of younger children were aware of the need for immunizations against childhood diseases. The survey found that the median age at which parents believe all Hib immunizations should be completed is three years, but 30% could not answer the question. (The Hib series should be completed by age two years.) It is especially important that the immunization message be forcefully delivered this year. The IAP will no longer be funded after December, 1996. Because of this, it is difficult to know what the role of the Weld County Children's Immunization Coalition will be after this year. INCREASE IN THE NUMBER OF YOUTH ANTICIPATED IF MONEY IS RECEIVED. (cont.) However, at least 3,242 immunizations are projected to be given at Weld County Health Department clinics during 1996. • 960108 WELD COUNTY CHILDREN'S IMMUNIZATION COALITION GRANT APPLICATION Project Title Weld County Children's Immunization Coalition Public Relations Proiect Applicant's Name Weld County Children's Immunization Coalition Address 1517 16 Avenue Court Greeley, CO 80631 Phone {970) 353-0586 Director John S.
