RE: APPROVE REVISION TO NOTIFICATION OF FINANCIAL ASSISTANCE AWARD
FOR PAC FUNDS WITH DIVISION OF HUMAN SERVICES AND AUTHORIZE CHAIR
TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Revision to Notification of Financial
Assistance Award for PAC (Placement Alternatives Commission) Funds between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Social Services, and the Weld County Division of Human
Services, commencing January 1, 1996, and ending September 30, 1996, with further terms
and conditions being as stated in said revision to notification, and
WHEREAS, after review, the Board deems it advisable to approve said revision to
notification, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Revision to Notification of
Financial Assistance Award for PAC Funds between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department
of Social Services, and the Weld County Division of Human Services be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said revision to notification.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 1st day of July, A.D., 1996, nunc pro tunc January 1, 1996.

SUBJECT: Revision to the Notification of Financial Assistance Award with the Weld County
Division of Human Services under the Weld County Family Support Program
The Weld County Placement Alternatives Commission and the Weld County Department of
Social Services recommend Board approval of a revision to the Notification of Financial
Assistance Award (NFAA) with the Weld County Division of Human Services under the Weld
County Family Support Program.
