FACTUAL RECITALS Authority exists in the Law and Funds have been budgeted, appropriated and otherwise made available and a sufficient uncommitted balance thereof remains available for encumbering and subsequent payment of this contract under Encumbrance Number PPG950409 in Fund Number 100, Appropriation Account 773, and Organization 7340. Required approval, clearance and coordination has been accomplished from and with appropriate agencies: and The parties entered into a contract dated August 1, 1994, more fully described below. This contract is being amended to provide program standards for submission of data, follow-up required for women with abnormal results, and include funding for a Case Manager, that is predicated on meeting the new program standards and budget established with this amendment. NOW THEREFORE, it is hereby agreed that for and in consideration of their mutual promises to each other, hereinafter stated, the parties hereto agree as follows: 1. Consideration for this Amendment to the Original Contract, PPG 950409 , Contract Routing Number 950409, dated August 1. 1994, consists of the payments which shall be made pursuant to this agreement and the promises and agreements herein set forth. 2. It is expressly agreed by the parties that this contract is supplemental to the Original Contract Number PPG 950409, Routing Number 950409 dated August 1. 1994, referred to as the "Original Contract" which is by this reference, incorporated, made a part hereof as Exhibit _, and all terms, conditions and provisions thereof, unless specifically modified herein, are to apply to this Amendment as though they were expressly re -written, incorporated and included herein. 3. It is agreed that the Original Contract is and shall be modified, altered, and changed in the following respects only: y Page 1 of h Pages a) By establishing in the Original Contract Paragraph 6, the following maximum amounts for women aged 40 and above for the next fiscal year beginning August 1, 1995 and ending July 15, 1996: "Eight -teen Thousand Four Hundred and Seventy One Dollars ($18,471.00) for breast and cervical cancer screening for women aged 40 and over for the next fiscal year beginning August 1, 1995 and ending July 15, 1996. A copy of the new budget is attached and by this reference incorporated herein as Exhibit L." b) By adding to the original contract PPG 950409, Paragraph 13 as follows: " Ia. Funding for a CIS,' Manager is being provided in the annual budget (Exhibit L). This funding is to facilitate case enrollment, data completion, and data submission to the CPCP. This funding is based on $50 per case based on the number of budgeted screening cases and consistent adherence to the following standards of performance. Failure to accept funding for the Case Manager does not exempt the Contractor from meeting these compliance standards. • C -ace enrollment forms must be submitted to the program for 90% or more of the cases within 45 days of the dates of service. • Case enrollment forms must be submitted with essential information properly completed in 90% or more of the cases. • Mammogram results must be submitted to the CPCP in 90% or more of the cases within 45 days of the service being provided. This information must be submitted on the CPCP designated form or approved dictated reports from the mammography center which include all required data elements. • Mammogram results must have the essential information reported properly and accurately in 90% or more of the cases submitted. • Cases involving screening breast or cervical abnormalities must have diagnostic follow-up information provided to the CPCP in 90% or more of the cases within 60 days from the date of detection of the abnormality. This includes women who are referred to outside health care providers for diagnostic procedures. • Breast and Cervical Diagnostic Follow-up Reports must be submitted with essential information properly completed in 90% or more of the cases. • Women with screening abnormalities will be required to have either completed diagnostic follow-up or have documentation of lost to follow-up submitted to the CPCP on every case (100%). A copy of the documented follow-up efforts must be submitted to the CPCP with the appropriate diagnostic follow-up form. The minimum follow-up effort accepted will be one registered/certified letter, but could include telephone and regular mail contacts. • Planned diagnostic follow-up procedures must be completed and submitted to the CPCP in 90% of the cases within 45 days of the planned date. The completed procedure does not have to be the same as the one planned, but follow-up must be completed in the recommended timeframe to allow for continuity of care. Page 2 of 4 Pages Hello
