RE: APPROVE CONTRACT AMENDMENT NO. 1 TO MINIMUM DATASET CONTRACT
BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Amendment No. 1 to the
Minimum Dataset Contract between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health Department,
and the Colorado Department of Public Health and Environment, commencing March 1, 1996, and
ending February 28, 1997, with further terms and conditions being as stated in said contract
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said contract
amendment, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Amendment No. 1 to the Minimum Dataset Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department of
Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said contract amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 22nd day of May, A.D., 1996, nunc pro tunc March 1, 1996.

CONTRACT AMENDMENT NO. 1
THIS AMENDMENT, made this 1st day of March, 1996, by and between the State of Colorado
for the use and benefit of the Colorado Department of Public Health and Environment. 4300
Cherry Creek Drive South, Denver, Colorado 80222-1530 hereinafter referred to as the State,
and Weld County Health Department, 1517 16h Ave. Ct. Greeley, CO 80631„hereinafter
referred to as the contractor.
FACTUAL RECITALS
Authority exists in the Law and Funds have been budgeted, appropriated, and otherwise made
available and a sufficient unencumbered balance thereof remains available for payment in Fund
Number 100, Appropriation Code F86, Contract Encumbrance Number ADM 960536 FAA; and
Required approval, clearance andcoordination have been accomplished from and with appropriate
agencies; and
The parties entered into a contract dated October 1, 1995 with contract encumbrance number
ADM 960536 contract routing number 9650536, hereinafter referred to as the"Original
Contract". The Original Contract is attached hereto as Exhibit A and incorporated herein by the
reference.
The Original Contract is being amended to change the start and stop dates of the Original
Contract because of the delay in funding.
The contractor has given written assurance that no services were performed prior to the effective
date of this amendment. A copy of this written assurance is attached hereto as Exhibit B and
incorporated herein by this reference.
NOW THEREFORE, it is hereby agreed that:
1. Consideration for this Amendment to the Original Contract, CE Number ADM 960536,
Contract Routing Number 960536, dated October 1, 1995, consists of the payments which shall
be made pursuant to this Amendment and the promises and agreements herein set forth.

2. It is expressly agreed by the parties that this Amendment is supplemental to the Original
Contract CE Number ADM 960536, Contract Routing Number 960536, dated October 1, 1995,
which has been incorporated as Exhibit A, and all terms, conditions, and provision thereof, unless
specifically modified herein, are to apply to this Amendment as though they were expressly
rewritten, incorporated, and included herein.
3. It is agreed the Original Contract is and shall be modified, altered, and changed in the
following respects only:
a. The parties hereby agree that the Original Contract shall commence on March1,
1996, and shall end on February 28, 1997.
b. The six and twelve month reports described in paragraph 2. of the Original
Contract shall be due on November 15, 1996, and March 15, 1997, respectively.
4. The effective date of this Amendment is March 1, 1996.
5. Except for the"Special Provisions" and "General Provisions," in the event of any conflict,
inconsistency, variance, or contradiction between the provisions of this Amendment or any of its
attachments or exhibits, and any of the provisions of the Original Contract, the provisions of this
Amendment shall in all respects supersede, govern, and control. The"Special Provisions" and
"General Provisions" shall always be controlling over other provisions in the contract or
amendments. The factual representations in the Special Provisions concerning the absence of
bribery or corrupt influences and personal interest of State employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT
FISCAL YEAR ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING
APPROPRIATED, BUDGETED, AND OTHERWISE MADE AVAILABLE.
7. THIS AMENDMENT SHALL NOT BE DEEMED VALID UNTIL IT SHALL HAVE
BEEN APPROVED BY THE CONTROLLER OF THE STATE OF COLORADO OR SUCH
ASSISTANT AS HE MAY DESIGNATE.

Subject: Status Report - PHHS Block Grant Minimum Dataset Project
Date: April 15, 1996
This will respond to Kelly Pearson's memorandum of March 13, 1996 regarding status reports for
the Preventive Health&Health Service Block Grant Projects.
1. Project Funding Cycle
Due to the uncertainty of the CDC's funding of the project, Weld County delayed
implementation until March 1, 1996 and incurred no expenditures prior to that date.
We therefore are requesting a annual funding cycle of March 1, 1996 through February
28, 1997 accordingly.
2. Detailed Accounting of Project Expenditures
Attached is a dam.tailed,accounting of expenditures for the duration of the project through
March 31, 1996. The report also includes the revised budget to reflect the 8.6% decrease
in the original contract and the anticipated expenditures through July 1, 1996.
Our expenditures, through March 31, account for 14% of budgeted annual expenditures
due to the one time expense of the computer. The anticipated projections through
June 30, 1996 appear to be commensurate with the annualized budget. You will note that
75% of projected expense is in personnel line items (wages &benefits) and therefore are
not customarily subject to much variance. The remaining line items include direct
administrative expenses for the project and will be incurred as the project reaches
specific objectives such as publishing the Minimum Dataset(printing), purchase of data, if
necessary, conducting training sessions, etc. and therefore will not necessarily be incurred
with any periodicity.

3. Project Summary
Staffing:
Due to the uncertainty of CDC's funding of the PH&HS Block Grants, Weld County
Health Department delayed implementation of the project until March 1, 1996, when
CDC's budget for FFY 1996 was finalized.
Effective March 11, 1996, Chuck Morrison was employed to fill the Minimum Dataset
Specialist position. Chuck's background is in the environmental sciences and computer
information systems and he was formerly an employee of Weld County Information
Services. Chuck's computer science background is a definite asset to the project as we try
to automate the compilation and dissemination of the Minimum Dataset and other health
related information.
Annual Objectives:
(1) Assemble state wide databases for inclusion in the Minimum Dataset for Colorado
Counties.
Databases are currently being compiled for inclusion into the Minimum Dataset.
Databases from the Division of Heath Statistics, Cancer Registry, Division of
Epidemiology, as well as, the departments of Education and Social Services have
been accessed in electronic format and are being processed. The Local Health
Data Users Group is serving in an advisory capacity to the project.
(2) Disseminate annually to all counties the Minimum Health Dataset for Colorado
Counties.
Although it is too early in the project to anticipate when the Dataset will be
disseminated, we hope to achieve a more timely dissemination of the reports and
expand dissemination to include the electronic media and formats.
(3) Develop training materials and strategies to assist local health agencies to collect
data locally and to enable them to use available infrastructure , technology and
resources to their maximum benefit locally.
A survey to determine the capacity of local health departments and nursing
services to utilize databases electronically as well as, their ability to communicate
with each other electronically has been sent out and the results are currently being
compiled. Two additional surveys are to be designed and sent out to determine
training needs and additions to minimum dataset indicators. The results of these
surveys will be used in part to determine formalized training programs to be
conducted during the project period. The Minimum Dataset Project is
collaborating with other PH&HS Block Grant Projects in our training efforts.

(4) Develop Systems for the transfer of data through electronic communications including
CDC Wonder and / or the Internet.
The objective of the survey referenced and which is attached above was to
determine in part the capacity of local health departments and nursing services to
meet this objective and to ascertain training and hardware that might be required to
implement electronic transfer universally.
In summary, considerable achievements have been made toward achieving the goals of the
project in a relatively short period of time. Weld County Health Department remains excited
about the outcomes of the project and to establishing new collaborative efforts in order to expand
the databases included in the Minimum Dataset.

Amendment#1 to the Minimum llatacet Contract
Enclosed for Board approval is Amendment Number One to the Minimum Dataset Contract for
Preventive Block Grant Funding.
In December 1995, the Board approved the contract which will fund a full-time staff member
who will collect minimum health indicator data. This data will then be shared with other
organized Health Departments throughout the State for the purposes of measuring health
problems and for program planning.
The contract is now being amended to change the start and stop dates because of the delay in
funding. The original dates were from October 1, 1995 through September 30, 1996. The
amendment provides for the contract to commence on March 1, 1996 and end on February 28,
1997.
I recommend your approval of this amendment.
