I . BY THE COMMISSION:
Statement
1 . The Commission is aware that recently, due to var-
ious events, components of the 911 system operated by U S WEST
Communications, Inc. ("USWC") , and other local exchange providers
in various areas have suffered significant failure. That is,
there have been recent specific instances in certain areas of the
state in which calls could not be placed on the 911 system in
USWC' s service territory. One such event occurred on Septem-
ber 24, 1998 in the Denver Metro area. Due to that failure, many
residents in portions of Denver and Pitkin Counties were unable
to place 911 calls for up to 14 hours while other 911 calls were
transported to the agencies responsible for answering the calls
without the caller identification information that normally
accompanies such calls. Additionally, the Commission recently
learned through the public media, that on October 7, 1998, a

severed USWC phone cable left a large portion of Elbert County
without 911 service for several hours.
2 . In order to investigate these and potential future
failures in the 911 system (e. g. , to investigate potential single
points of failure on the USWC system which could render the 911
system inoperable) and to address potential solutions to these
problems, the Commission will open the instant docket . Commis-
sion Staff will be directed to conduct an investigation into
these matters and will file a report in this docket for con-
sideration by the Commission.
II . ORDER
A. The Commission Orders That:
1 . The instant docket is opened for the purposes
stated above.
2 . Staff of the Commission is directed to conduct an
investigation of the matters discussed here and shall file a
report setting forth its conclusions and recommendations, such
report to be filed in as expeditious a manner as possible, as
determined by Staff.
3 . This Order is effective on its Mailed Date.
