RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT WITH ACKERMAN
AND ASSOCIATES, P.C., AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Child Protection
Agreement between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Social Services, and
Ackerman and Associates, P.C., with terms and conditions being as stated in said amendment,
and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to Child
Protection Agreement between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Ackerman and Associates, P.C., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of April, A.D., 1998.

AMENDMENT TO CHILD PROTECTION AGREEMENT
BETWEEN WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND ACICERMAN AND ASSOCIATES, P.C.
THIS AMENDMENT made and entered into this 20th day of April, 1998, by and
between the Weld County Department of Social Services, hereinafter referred to as "Social
Services," and the Ackerman and Associates, P.C., hereinafter referred to as "Contractor"
WITNESSETH
WHEREAS, the parties desire to modify their current Agreement for Child Protection
Services (PY98-CPS-5) in order to increase the hours and funding per family participating in
Family Group Conferencing; and
WHEREAS, the authority existing in the Law and Expedited Permanency Planning
Funds have been budgeted, appropriated, and otherwise made available; and
WHEREAS, required approval, clearance, and coordination have been accomplished
from and with appropriate agencies.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and
agree as follows:
1. Exhibit B, Payment Schedule, Item 2., Fees for Services, will be revised as follows:
Social Services agrees to pay the Contractor at the rate of$75 per hour up to a maximum
of 25 hours per family or a total maximum of$1,875.00 per family.
Except as modified herein, the terms and conditions of the original agreement PY98-CPS-5,
together with all attachments of the original Agreement, remain in full force and effect.
IN WITNESS WHEREOF, the parties hereto have duly executed the Modification as the
day, month, and year first above written.

Enclosed for Board approval is an Amendment to a Child Protection Agreement between the
Weld County Department of Social Services and Ackerman and Associates, P.C.
The purpose of this Amendment is to increase the number of hours and fees for Ackerman and
Associates as follows:
1. conduct Family Group Conferencing under Expedited Permanency Planning from 20
hours to 25 hours per family.
2. increase the total maximum fee from $1,500 to $1,875 per family.
The hourly rate of$75.00 and the total budget under the Agreement remain the same as provided
under the original agreement.
