RESOLUTION
RE: APPROVE CHANGE ORDER LETTER NO. 1 BETWEEN COLORADO DEPARTMENT
OF PUBLIC HEALTH AND ENVIRONMENT, DISEASE CONTROL AND
ENVIRONMENTAL EPIDEMIOLOGY DIVISION, AND HEALTH DEPARTMENT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter No. 1 between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, and the Colorado Department of Public
Health and Environment, Disease Control and Environmental Epidemiology Division, commencing
January 1, 1996, and ending December 31, 1996, with further terms and conditions being as stated
in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Change Order Letter No. 1 between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Health Department, and the Colorado Department of Public Health and Environment,
Disease Control and Environmental Epidemiology Division, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 24th day of April, A.D., 1996, nunc pro tunc January 1, 1996.

Change Order Letter No. 1
In accordance with Paragraph 17 of routing number 96-0643, of contract number SOD960643, (copy
attached and by this reference made a part hereof) between the State of Colorado Department of Public
Health and Environment (Disease Control and Environmental Epidemiology Division) and Weld County
Health Department covering the period of January 1, 1996 through December 31, 1996 the undersigned
agree that the maximum amount payable by the State for eligible services in Paragraph -1-5 is being
increased by $3723 to a new total of $4684. The first sentence in Paragraph 15 is hereby modified
accordingly. The terms and conditions of the original contract shall remain the same.
This amendment to the contract is intended to be effective as of 5/1/96, but in no event shall it be deemed
valid until it shall have been approved by the State Controller or such assistant as he may designate.

Subject: Change Order Letter No. 1 for HIV Counseling and Testing

Enclosed for Board approval is Change Order Letter No. 1 for the HIV Counseling and Testing
Services Contract between Weld County Health Department and the Colorado Department of
Public Health and Environment.
Earlier this year, the Board approved this contract for the amount of$961 which was 20% of the
anticipated budget for this year. Because the Federal Government has now approved the HIV
Prevention budget,this change order letter is being issued to increase the amount of the contract
by $3,723 to bring it to a total of$4,684 for the period January 1, 1996 through December 31,
1996. This is an increase of$634 over last year's finding.
I recommend your approval of this contract.
