RE: APPROVE AMENDMENT TO WELD COUNTY JUVENILE COMMUNITY REVIEW
BOARD BYLAWS, PROCEDURES AND CRITERIA AND APPOINT TWO MEMBERS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, by Resolution dated March 7, 1994, the Board approved revised bylaws for
the Weld County Juvenile Review Board ("Review Board"), with an amendment thereto being
approved by the Board by Resolution dated August 17, 1998, and
WHEREAS, the Board of County Commissioners now desires to amend said bylaws to
provide for the appointment of alternate members and to provide a clearer procedure for the
amendment of the bylaws by the Board of County Commissioners.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Weld County Juvenile Community Review Board Bylaws,
Procedures and Criteria, approved March 7, 1994, and amended August 17, 1998, be, and
hereby are, amended to provide for the appointment of alternate members and to provide a
clearer procedure for the amendment of the bylaws by the Board of County Commissioners.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 13th day of September, A.D., 1999.

COLORADO
Attached is various correspondence which we have had with the Juvenile Community Review Board since
April of this year. As you can see in my April 8, 1999, letter and attachments thereto, it was suggested that
the Review Board be expanded to include alternates and to require that amendments be approved by the
Board of County Commissioners.
I met with the Juvenile Community Review Board on Thursday, August 5, 1999. We spoke about that
contents of my April 8, 1999, letter. The Review Board asked if it would be possible to have an alternate be
designated for each Board member. The reason is they feel strongly they need to have their designated
positions represented in any decisions made by the Review Board.
It makes sense to me to have each member be responsible to designate and communicate with an alternate.
The alternates could be approved by the Board of County Commissioners. Recently the Review Board has
had problems achieving a quorum. My thought is that having each member in close communication with his
or her alternate would be an advantage, because the alternates would know when to appear so the Review
Board could achieve a quorum.
The Review Board had no objection to the change such that the Board of County Commissioners is the
approving authority for the bylaws.
I recommend that the Board approve a change to the bylaws such that each member has an alternate. The
alternates will need to be approved by the Board of County Commissioners.

WELD COUNTY JUVENILE COMMUNITY REVIEW
BOARD BYLAWS, PROCEDURES AND CRITERIA
NAME
The name of this entity shall be the Weld County Juvenile Community Review Board,
hereinafter referred to as the "Board."
PURPOSE
The purpose of the Board shall be to carry out the intent and purposes of the Board of County
Commissioners of Weld County pursuant to the resolutions of November 2, 1987, and November
4, 1987, concerning the creation of the Juvenile Community Review Board and the appointment of
its members, requiring approval by the Board of residential community placements within Weld
County of juveniles committed to the Department of Institutions in accordance with C.R.S. 19-2-
1301, et seq.
ADDRESS
The address for the Board shall be c/o Weld County Juvenile Community Review Board,
P.O. Box A, Greeley, Colorado 80632.
ADMINISTRATIVE PROCEDURES
A. Officers and Duties
1) The Board shall elect a Chairperson, Vice Chairperson and Secretary, each
of whom shall serve for a one year term to begin in November. The Chairperson
shall preside at all meetings of the Board and act in accordance with the duties
customarily associated with such position. The Vice Chairperson shall act in the
absence or at the request of the Chairperson.
2) Record Keeping Duties:
The Chairperson will provide Agendas in advance of meetings when there is
business to be conducted. A summary of the actions and decisions of business
meetings will be provided by the Secretary, for correction and approval by the
Board.

B. Meetings
1) All business of the Board shall be transacted at regular meetings or special
meetings and only when a quorum is present. Regular meetings shall be conducted
monthly.
2) Special meetings may be held subject to the call of the Chairperson or 2 or more
appointed members to act upon matters that cannot be delayed until the next
regular meeting date.
C. Quorum
1) A quorum shall consist of 5 of the nine members (or designated alternates) in
attendance to conduct official business. Only appointed Board members Of
their dsignutt.d alternates a'may vote. Any tie vote
shall be construed as a negative vote.
2) The Chairperson or acting Chairperson is a regular voting member.

Commissioners of Weld County. Membership represents the following areas:

B. Social Services
C. School District
C. Law Enforcement
D. Division of Youth Services
E. Bar Association
F. Mental Health
G. Citizen
H. Citizen
I. Person recommended by the Juvenile Services Planning Commission, a/Ida,
Senate Bill 94 Planning Committee.

of the entire membership of the Board provided seven (7) days notice has been given to each

CONFIDENTIALITY
All names,addresses,and information regarding a child reviewed by the Board shall be
confidential and not disclosed except to the Board or its designees without the express written
permission of the child and the legal custodian.
BOARD REVIEW CRITERIA
The Board, in considering the criteria and scope of review, shall review information
including, but not limited to:
1) Whether the juvenile is appropriate for community placement based upon presentation
by the Client Manager and relevant documentation including the DOI/DYS risk
assessment.
2) The placement of the juvenile in Weld County will not present a danger to the safety and
welfare of the public.
3) The DOI/DYS has appropriate and sufficient arrangements with community resources
to provide the treatment program proposed.
4) The affected local school district(s)receive from DOI/DYS andthe school
district(s) previously involved complete educational, psychological, and social
information needed by the local district in a timely manner.
5) Community placement of the juvenile offers beneficial outcome compared
to institutional placement.
6) Other relevant factors of the juvenile's situation to justify community placement.
7) Documents to be reviewed shall include, but not be limited to:
--the referral form
--probation report(s)

--probation risk/need assessments
--family interview
--vocational assessment
--district attorney assessments
--clinical interviews and evaluations
--institutional progress reports
--DOI/DYS continuity care plan
--Social Services placement evaluation(s), update(s), and report(s)
--DOI/DYS risk assessment
--drug and alcohol evaluation(s)
--placement history, including complete run history
OTHER RELATED CRITERIA
1) Should information about the juvenile or the community placement upon which
the Board based its decision, in whole or in part,be inaccurate or, false,the Board may
rescind approval of the placement made in Weld County.
2) The Client Manager will present the case and be available to answer questions
or provide additional information as needed.
3) The Client Manager shall. inquire about and inform the Board about any
pending charges against the child.
4) The DOI/DYS and the community placement shall provide updated information to the
Board about the placement and placed child after placement has occurred as requested
by the Board.
5) Failure to provide documents or information requested by the Board may result in denial
of approval for placement in Weld County.
6) The names and numbers for 24 hour information and notification for persons involved
with the child from the placement and DOI/DYS notification should be available to the
Board Chairperson or his designee.
7) The contract provider and DYS shall report to the Chair of the Board and/or the D.A.'s
office, all escapes/A.W.O.L.'s (aswell-as to the police) and any other delinquent or
criminal offenses within 72 hours, including weekends and holidays.
8) It is the policy of this Board that any juvenile who escapes or is A.W.O.L. must be re-
referred to the Board for placement back in the community if the escape/A.W.O.L was
seven (7) days or longer.

9) All contract providers shall provide to the Board written reviews of each juvenile placed
with the provider for the previous month, or any portion of the month. These reports
shall be delivered to the Chair of the Board not less than three (3) days prior to the
regularly scheduled meeting.
The Board referral form shall be mailed by DOI/DYS simultaneously to the official address
of the Board and the chairperson mailing address.
