RE: APPROVE CHANGE ORDER LETTER#1 FOR CHLAMYDIA TESTING CONTRACT
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 for the
Chlamydia Testing Contract between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
January 1, 1997, and ending December 31, 1998, with further terms and conditions being as
stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Change Order Letter#1 for the Chlamydia Testing Contract between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, and the Colorado Department of
Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of November, A.D., 1998, nunc pro tunc January 1, 1997.

Change Order Letter#1 to the Chlamydia Testing Contract
Enclosed for Board review and approval is Change Order Letter#1 to the Chlamydia Testing
Contract between Weld County Health Department and the Colorado Department of Public
Health and Environment.
This letter will provide an additional reimbursement of$2,145.00 for chlamydia tests done in our
sexually transmitted disease and family planning clinics. The additional funding will bring the
total amount of the contract to $8,895.00 for the two-year period January 1, 1997 through
December 31, 1998.

Change Order Letter No.
In accordance with paragraph 4 of the contract with routing number 97-06275 and contract encumbrance number
P19706275,(as amended by Amendment fi 1,routing number 98-07899), hereinafter referred to as the"Original
Contract"(a copy of which is attached hereto and by this reference made a part hereof)between the State of
Colorado,Department of Public Health and Environment and Weld County Health Department,covering the
period of January 1. 1997 through December 31. 1998,the parties agree that the maximum amount payable by the
State for the eligible services referenced in paragraph 3 of the Original Contract is being increased by TWO
THOUSAND ONE HUNDRED FORTY FIVE DOLLARS (52.145.001 to a new total of EIGHT THOUSAND
EIGHT HUNDRED NINETY FIVE DOLLARS (58.895.001 to reimburse the Contractor for Chlamydia Testing
lab fees incurred during the contract period. The first sentence in paragraph 2 of the Original Contract is hereby
modified accordingly. All other terms or conditions of the Original Contract arc hereby reaffirmed.
This amendment to the Original Contract is intended to be effective as of October 15. 1998,but in no event shall it
be deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.
