HomeMy WebLinkAbout960106.tiff RESOLUTION RE: APPROVE CONTRACT RENEWAL LETTER NO. 2 BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT (DISEASE CONTROL AND ENVIRONMENTAL EPIDEMIOLOGY DIVISION, COMMUNICABLE DISEASE SECTION) AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Contract Renewal Letter No. 2 between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and the Colorado Department of Public Health and Environment (Disease Control and Environmental Epidemiology Division, Communicable Disease Section), commencing January 1, 1996, and ending December 31, 1996, with further terms and conditions being as stated in said contract renewal letter, and WHEREAS, after review, the Board deems it advisable to approve said contract renewal letter, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Contract Renewal Letter No. 2 between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and the Colorado Department of Public Health and Environment (Disease Control and Environmental Epidemiology Division, Communicable Disease Section) be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized to sign said contract renewal letter. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 24th day of January, A.D., 1996, nunc pro tuns January 1, 1996. This contract pays for chlamydia tests done in the sexually transmitted disease and family planning clinics. This current renewal is written for only $735 which is 20% of predicted funding for the year. This is due to the federal budget delay. As the federal budget is resolved, this contract will be amended to reflect the increased funding. If the projected funding is stable, the contract will be approximately $562 larger than last year's contract. I recommend your approval of this renewal letter. Enclosures 960106 Hello
