RE: APPROVE CONTRACT RENEWAL LETTER NO. 2 BETWEEN HEALTH DEPARTMENT
AND COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT (DISEASE
CONTROL AND ENVIRONMENTAL EPIDEMIOLOGY DIVISION, COMMUNICABLE
DISEASE SECTION) AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Renewal Letter No. 2 between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, and the Colorado Department of Public
Health and Environment (Disease Control and Environmental Epidemiology Division, Communicable
Disease Section), commencing January 1, 1996, and ending December 31, 1996, with further terms
and conditions being as stated in said contract renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said contract renewal
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Renewal Letter No. 2 between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld
County Health Department, and the Colorado Department of Public Health and Environment
(Disease Control and Environmental Epidemiology Division, Communicable Disease Section) be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said contract renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 24th day of January, A.D., 1996, nunc pro tuns January 1, 1996.

Contract Renewal Letter No. 2
In accordance with Paragraph 3 of contract routing number 94-0876 of Contract No. EPI940876
and routing number 95-1523 of Letter No. 1 (copies attached and by this reference made a part
hereof as Attachment A) between the State of Colorado Department of Public Health and
Environment (Disease Control and Environmental Epidemiology Division, Communicable
Disease Section) and Weld County Health Department covering the period of January 1, 1996
through December 31, 1996 the undersigned agree that the maximum amount payable by the
State for eligible chlamydia testing services is'$735 to be spent according to the budget outlined
below. Funding is contingent upon continued availability of federal funds for payment pursuant
to the terms of this agreement. The terms and conditions of the original contract shall remain
the same.
Family Planning Clinic chlamydia tests
(48 @ $7.50 each) $ 360
STD Clinic chlamydia tests
(women and male contacts)
(50 @ $7.50 each) $ 375
TOTAL $ 735
This amendment to the contract is intended to be effective as of 01/01/95, but in no event shall
it be deemed valid until it shall have been approved by the State Controller or such assistant as
he may designate.

Enclosed for Board review and approval is a renewal letter for the Chlamydia Testing
contract between the Weld County Health Department and the Colorado Department of
Public Health and Environment.
This contract pays for chlamydia tests done in the sexually transmitted disease and
family planning clinics. This current renewal is written for only $735 which is 20% of
predicted funding for the year. This is due to the federal budget delay. As the federal
budget is resolved, this contract will be amended to reflect the increased funding. If the
projected funding is stable, the contract will be approximately $562 larger than last
year's contract.
I recommend your approval of this renewal letter.
