RE: APPROVE AGREEMENT FOR PROVISION OF INTERNSHIP EXPERIENCES
BETWEEN HEALTH DEPARTMENT AND UNIVERSITY OF NORTHERN COLORADO
AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement for Provision of
Internship Experiences between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the University of Northern Colorado, commencing June 1, 1996, and ending
June 1, 1999, with further terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Agreement for Provision of Internship Experiences between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the University of Northern
Colorado, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of August, A.D., 1996, nunc pro tunc June 1, 1996.

AGREEMENT FOR PROVISION OF INTERNSHIP EXPERIENCES
BETWEEN
UNIVERSITY OF NORTHERN COLORADO
AND
WELD COUNTY HEALTH DEPARTMENT
This Agreement is made between the BOARD OF TRUSTEES OF THE UNIVERSITY OF
NORTHERN COLORADO, Greeley, Colorado, hereinafter referred to as "UNC," and Weld
County Health Department, 1517 16th Avenue Court, Greeley, Colorado 80631, herinafter
referred to as "AGENCY."
WHEREAS, UNC has established education programs, herinafter referred to as
"Programs" and indentified in Attachment A hereto, which require the educational
facilities of the Agency,
WHEREAS, clinical or internship education and experience is a required and integral
component of the University Program, and
WHEREAS, UNC desires the assistance of the Agency in developing and implementing
education for its students,
NOW,therefore, in consideration of the mutual agreements set forth herein, the Agency
and UNC agree as follows:
1. RESPONSIBILITIES OF UNC
1.1 UNC will assume full responsibility for planning and implementation of the
education program of the student.
1.2 UNC will refer to the Agency only those students who have satisfactorily
completed prerequisite clinical and/or didactic portion of the curriculum.
1.3 UNC will identify a person or persons to direct the internship educational
program at the Agency and to act as liaison for theAgency, UNC and the
student(s).
1.4 UNC will be responsible for the determination of a student's final grade.
1.5 UNC will notify the Agency of its planned schedule of student assignments,
including the dates and purpose of the internship experience, the name(s) of the
student(s), and the level of academic preparation. All schedules and assignments
must be acceptable to the Agency.
1.6 UNC will provide the Agency with objectives for individual internships (or
specialized classes) and evaluation forms for the Agency to provide input for the
evaluation process.

1.7 UNC will maintain ongoing communication with the Agency on items pertinent
to internship education. Such communication should include, but not be limited
to, on-site visits or phone conferences with the Agency and the provision of
educational materials relevant to the educational program.
1.8 UNC will advise students and faculty assigned to the Agency of their
responsibility for complying with the policies, rules and regulations of the
Agency.
1.9 UNC shall maintain all educational progress records pertaining to its students.
1.10 The students will maintain health insurance or be responsible for all medical
expenses incurred during a clinical or internship experience. UNC expressly
certifies that it is an agency of the State of Colorado and that the activities
contemplated by this contract, and the participation of its students, employees and
agents, are entitled and subject to the operation of the Colorado Governmental
Immunity Act, C.R.S. 24-10-011, et.seq., (1988 as amended), and are covered for
liability by the Division of Risk Management of the State of Colorado.
Documentation of this coverage may be requested in writing by the agency.
1.11 The University is covered by worker's compensation insurance with the State
compensation Insurance Fund. This coverage is provided to those students who
receive no pay or remuneration and are enrolled in required on-the-job training
programs, except student teaching.
2. RESPONSIBILITIES OF THE AGENCY
2.1 The Agency will have ultimate responsibility for client/patient care or patron
service and that care or service is its first priority.
2.2 The Agency will provide the physical facilities, equipment, supplies, and patients,
clients or patrons to conduct the internship experience program in accordance
with the education objectives.
2.3 The Agency will provide each student with access to a planned supervised
program of internship experience, consistent with the educational objectives and
program established by UNC.
2.4 The Agency will provide a designated staff member to act as the internship
supervisor for student(s).
2.5 The Agency will provide the assigned student(s) with an orientation to the
Agency and provide each student with access to pertinent rules and regulations,
including applicable confidentiality laws, rules,regulations and procedures with
regard to patient, client or patron records.

2.6 The Agency will evaluate the performance of the assigned student(s) in writing,
using forms provided or approved by UNC.
2.7 The Agency will advise UNC in a timely manner of any serious deficiency noted
in an assigned student's performance. It will then be the mutual responsibilities of
the student, the Agency and UNC to devise a plan by which the student may be
assisted towards achieving the stated objectives of the internship experience.
2.8 The Agency has the right to require that UNC withdraw from the internship
experience program any student whose health(despite reasonable
accommodation) or performance is a detriment to patient, client or patron well
being or to the achievement of the stated objectives of the internship experience.
2.9 The Agency has the right to require that there be a termination of a student's
assignment if conditions at the Agency prove to be detrimental to student
learning.
2.10 The Agency shall be responsible for determining which of UNC's Programs shall
be eligible to participate in the benefits of the Agreement.
2.11 The Agency will advise UNC of any services available to the student.
3. MUTUAL RESPONSIBILITIES OF UNC AND THE AGENCY
3.1. UNC and the Agency will establish the internship education objective(s) for the
internship experience, devise methods for their implementation, and evaluate their
effectiveness.
3.2 UNC and the Agency will determine the number of students assigned to the
Agency and the length of the internship experience required to achieve the stated
educational objectives.
3.3 UNC and the Agency will not unlawfully discriminate against any employee,
applicant, or student enrolled in their respective programs because of race, color,
creed, religion, age handicap, sex, ancestry, veteran's status or national origin.
3.4 UNC and the Agency agree that no monies will be paid by either party to the
other under the terms and conditions of this Agreement and that the mutual
benefits contained herein accruing constitutes sufficient consideration herefor.
3.5 Both parties recognize that they are bound to comply with the Family Educational
Rights and Privacy Act(Buckley Amendment) in the handling of educational
records of students enrolled in their programs. It is also understood and
recognized that employees and agents of each party will need to have access to the
educational records maintained by the other party in properly administering their
duties and obligations under this agreement and to the individual students. It is

also agreed that each party shall thoroughly orient their employees and agents of
their obligations under the Family Educational Right and Privacy Act and shall
maintain their practices in strict accordance with the requirements of that Act.
Neither party shall be permitted to authorize any further disclosure of educational
records of students of the other party to persons or entities not a party to this
Agreement without first having received permission of the other party and having
obtained assurances that the other party has fully complied with the provisions of
the Family Educational Rights and Privacy Act. Any permitted disclosure to
persons or entities not a party to this Agreement shall be under the conditions that
no further disclosure by such parties shall be permitted.
3.6 This Agreement will become effective on June 1, 1996, and will remain in effect
until June 1, 1999, or as other provisions for termination contained herein may
allow. Revisions and modifications of this agreement may be made with mutual
consent, but must be in writing.
3.7 The parties to this Agreement intend that the relationship between them
contemplated by this Agreement is that of independent entities working in mutual
cooperation. University faculty, staff, and students are not to be considered
County employees for any purpose, and said persons may not hold themselves out
as employees or agents of Agency or otherwise make any representation or
commitment on behalf of Agency. Likewise, Agency staff are not to be
considered employees of UNC for any purpose, and said staff may not hold
themselves out as employees or agents of UNC or otherwise make any
representations or commitments on behalf of UNC. Agency shall in no way be
required to provide any pecuniary benefits, salaries, wages, or fringe benefits to
faculty, staff or students of UNC, and likewise, UNC shall in no way be required
to provide such pecuniary benefits to employees or agents of Agency.
3.8 It is expressly understood and agreed that enforcement of the terms and
conditions of this Agreement, and all rights of action relating to such
enforcement, shall be strictly reserved to the undersigned parties, and nothing
contained in this Agreement shall give or allow any claim or right of claim
whatsoever by any other person not included in this Agreement. It is the express
intention of the undersigned parties that any entity, other than the undersigned
parties, receiving services or benefits under this Agreement shall be deemed an
incidental beneficiary only.
THIS AGREEMENT contains the entire understanding of the parties and supersedes any and all
previous agreements. Either party shall have the right to terminate this agreement at any time,
having given the other party ninety days prior written notice of its intention to do so. Any
notices shall be sent by registered mail, return receipt requested, to the parties at the following
addressees.

Subject: Non-financial Agreement with University of Northern Colorado
for Clinical Experience and Education
Enclosed for Board approval is a non-financial agreement between Weld County Health
Department and the University of Northern Colorado (UNC).
The agreement outlines each agency's responsibilities regarding UNC students from the College
of Health and Human Science who will be receiving clinical experience at the Health
Department.
The term of this agreement is from June 1, 1996 through June 1, 1999. I recommend your
approval of this agreement.
