CHAIRMAN OF THE BOARD
TO SIGN AGREEMENT WITH
OTIS ELEVATOR CO, AND CONFIRMATION OF ELECTRIC CO,
WHEREAS, OTIS ELEVATOR CO, HAVE MADE AN AGREEMENT FOR THE
STEEL STRUCTURE OF THE ELEVATOR SHAFT at the Old Hospital, Weld County
Colorado, and also the confirmation from the Electric Co„ and
WHEREAS, the Board believes it to be to the best interests of the
County by the acceptance of said agreement.
NOW, THEREFORE, BE IT RESOLVED, by the Board of County
Commissioners of Weld County, Colorado that the Chairman of the board
be and he is authorized to sign agreement with Otis Elevator Co. for the
steel structure of the elevator shaft at the Old Hospital, Weld County,
Colorado and also the confirmation from the Electric Co.
The resolution was adopted by the following vote:

OPERATING DEVICES
In the Car: A flush type attractively finished metal panel which contains buttons marked to correspond
with the landings served, a light switch, an emergency-stop switch and an alarm button connected
to a bell which serves as an emergency signal.
At Hoistway Landings: A single push button at each landing. An IID-Use light jewel 'will be
provided in each push button fixture.
OPERATION
The car cannot be started unless the car door or gate is in the closed position and all hoistway doors
are locked in the closed position.
Momentary pressure of a car button will send the car to the landing selected where the car will auto-
matically stop. After a car button is pressed, uninterrupted use of the car will be insured until the desired
landing is reached and the car gate or door is opened and closed.
Momentary pressure of a landing button will bring the car to this landing unless the car is already
in use.
A time limit relay will be provided to render the car inoperative from the landing buttons for a few
seconds after a stop is made. This interval will be sufficient to permit a waiting passenger to open and close
the hoistway door and car gate or door and press a car button without interference from the landing buttons.

MACHINE: An Otis Single-Wrap Traction Geared Machine, especially designed for this service, will be
furnished. It will consist of an Otis motor, electro-mechanical brake, steel worm, bronze gear,
shaft and sheave, all compactly mounted on a single base or bedplate. Roller bearings will be
used for the sheave shaft supports, and all bearings will be provided of ample size and designed
for suitable lubrication.
The motor is especially designed for single-wrap traction machines and will be provided with
bearings arranged for efficient lubrication.
The brake will be spring applied and electrically released, and so designed as to contribute to
smooth stopping. The worm and gear will be made by a special process to insure long life with
minimum wear.

The driving sheave will be grooved for the proper number and size of hoist ropes and so
designed as to maintain constant traction.
SHEAVES AND BEAMS: Should car or counterweight sheaves be required, they will be of the best
grade of iron, turned true and grooved for the hoist ropes. The sheaves will be of ample diam-
eter and provided with heavy steel shafts mounted in suitable bearings. The sheaves will be
supported by steel beams or channels furnished and set in place by us.

PLATFORM AND CAR ENCLOSURE:
The platform will consist of a structural steel frame covered with
two layers of wood flooring. The top wood flooring will be T & G yellow
pine, and the underside of the platform will be covered with sheet steel
fireproofing. The platform will rest on rubber pads which are supported
on an auxiliary steel frame fastened to the car frame. This arrangement
foras an isolating cushion between the car and steel carframe. The
platform will be equipped with a metal threshold plate properly grooved
for the car door. We will provide a rubber tile floor covering of design
and color as selected, laid on asphalt felt, all securely cemented to the
top wood flooring.
Car enclosure will be of metal, car design #167, finished in colors
as selected. A two speed car door will be provided, mounted on rubber
tired hangers. Car to have stainless steel base, entrance columns, and
handrails.
LIGHT FIXTURE: A suitable light fixture will be furnished in the car and connected by us to a light outlet
located in the hoistway and provided by the purchaser. The location of the outlet should conform
to that shown on our drawings.
GUIDES: Steel tee guides will be furnished for the car and counterweight.
FOUNDATIONS: The machine will be placed directly over the
hoistway upon steel beams by us„ resting on supports
furnished in place by others.
ROPES: The elevators will be provided with suitable hoist and counterweight ropes.

firmly mounted at the bottom of the hoistway.

CAR FRAME: The car frame which supports the elevator platform and enclosure will be made of structural
jl steel members. It will be equipped with guide shoes and an Otis safety device. The hoisting ropes
+ supporting the car will be equipped with adjustable self-aligning hitches.
CAR SAFETY AND GOVERNOR: An Otis Car Safety will be mounted underneath the platform, and
arranged to stop the car in case it should attain excessive desrending speed. The Safety will be
i operated by a centrifugal speed governor, to which it is connected through a continuous steel rope.
The governor will be located at the top of the hoistway. Prior to the application of the Safety, all
power will be cut off from the motor and the brake applied.
j COUNTERBALANCE: The elevator will be provided with suitable counterbalance for smooth and economical
II operation.
TERMINAL AND FINAL LIMITS: The car will be slowed down and stopped automatically at the terminal
landings. Should the car travel beyond the terminal landings, the final hoistway limit switches will
I automatically cut off the power and apply the brake.
TERMINAL BUF.HERS: Buffers will be provided as a means for bringing the car and
counterweight to a positive stop at the extreme limits of travel, should the car for any reason run by
the final hoistway limit switches.
CONTROLLER: The controller will be of the Otis Electro-Magnetic Control Type, for use with Otis Electric
Traction Machines. The controller will contain electro-magnetic switches mounted on panels sup-
ported by an angle-iron frame. It is designed to control the speed of the motor and automatically
open the power supply, apply the brake and bring the car to rest in the event of the operation of any
of the safety devices, or failure of current from any cause.
OVERLOAD AND REVERSE PEASE RELAYS: An overload magnet relay and a reverse phase
relay will be provided on the controller.
PAINTING* We will paint all exposed metal work furnished by us under these speci—
fications except as otherwise specified herein.
ROLLER GUIDES* Rubber tired roller guides will be provided for car and counter-
weight. Rollers run on dry rails requiring no lubrication.

TYPE "O" ELECTRIC OPERATOR FOR CAR DOOR AND HOISTWAY DOORS: Provided for
it* elevators for opening and closing the car doors and the hoistway doors.
The engine on the elevator car operates the car door through suitable levers. When the elevator is
stopped at a floor, the car and hoistway doors operate simultaneously and are coupled together by a
rigid car door cam which engages a mechanism on the hoistway door.
The doors are power-opened and spring-closed but checked in both directions to eliminate slamming.
An electro-mechanical interlock on each hoistway door prevents movement of the elevator until all
hoistway doors are locked in the closed position.
An electric car door contact is provided which prevents elevator movement away from the landing
unless the door is in the closed position as defined in The American Standard Safety Code for Elevators,
Dumbwaiters and Escalators.
Necessary switches are in the elevator machine room to control the operation of the doors.
The doors open automatically when the elevator stops at a landing and close either after expiration
of a time interval or the moment a car button is pressed. Doors may he stopped and reversed during
their closing motion.
A protective device extends the full height and projects beyond the front edge of the car door.
Should this device touch a person or object while the car door is closing, the car and hoistway doors
return to the open position.
Should the electric power fail, the door operator is so designed that the doors may he manually
operated from within the car.

FRAMES: Otis Type E constructed of No. 14 U.S. gauge steel. The frame is securely fastened to the
sill and header and is returned on the hoistway side to present a neat appearance.
ANGLE STRUTS: Angle struts and closer supports extend from sill to building structure above and sup-
port the header.
DOORS: Flush type design, 1/" thick, made from No. 16 U.S.gauge steel and reinforced with interlocking
stiffener members. Doors include removable nonmetallic sill guides and are cushioned by rubber
bumpers or astragals.
HANGERS: Otis type S.
HEADERS: Constructed of s" formed steel which at each end is bolted to and supported by the strut
angles. The header provides support for the frame, support and housing for the hangers and forms
a lintel over the entrances. The lintel carries the load imposed by a 6" terra cotta wall provided
the span is limited to a maximum of 5'-0"and the ceiling height does not exceed 14'4)".
HANGER COVER PLATES: Made in sections of No. 14 U.S. gauge steel to provide hanger accessibility;
section over the door opening is removable from within the car.
ANTI-SLIP SILLS: Cast iron at ell floors Alit of the narrow typo.

fixed to the masonry or structure framing of the hoistway.
FASCIA PLATES: Provided at upper floors.
Formed with No. 14 U.S. gauge steel adequately reinforced which extends the width of the jamb
opening plus the jamb flanges and from the header to the nosing of the sill above.
DUST COVER: Provided at floor.
Made from No. 14 U.S. gauge steel and extends over the header the width of the jamb opening
plus the jamb flanges at the landing or landings for which fascia plates are not supplied.
TOE GUARD: Provided at first floor.
Made from No. 14 U.S. gauge steel and extends to the wall on a gradual bevel from the sill nosing.
SOUND DEADENING: Frames and doors include sound deadening material to minimize the transmittal
of noise.
HARDWARE: Bronze pulls are provided as needed for the hoistway or corridor side of the doors and service
or emergency keyways are supplied to conform with code requirements.
FINISH: Corridor side of doors— Prime coot. Owner to finish as desired.
Hoistway side of doors— Prize coat. Owner to finish as desired.
Frames— Prise coat. Owner to finish as desired.

WORK BY OTHERS: When hoistway walls must be erected before the frames are set,you agree to provide
rough hoistway openings as shown on the elevator layout to permit the installation of bucks, sills
and frames, to solidly build the surrounding walls into the frames and to thoroughly grout under the
sills under all conditions. After the frames are in position, you agree to protect them from pos-
sible damage during the construction period.

MAINTENANCE; Otis maintenance will be furnished on each elevator provided for under this contract for a
period of three months commencing on the date the elevator is turned over to you for use.
All work will be performed during our regular working hours of our regular working days. It will
consist of regular examinations and any necessary adjustment and lubrication of the equipment by
competent employees under our direction and supervision. The required supplies and parts will be
furnished except such parts as may be needed because of negligence, misuse or accidents not caused
by us. Upon your request, special examination will be made should trouble develop between regular
examinations and you agree to notify us promptly of any such trouble. No work or service other than
that specifically mentioned is included or intended.
It is agreed that we assume no liability for injuries or damage to persons or property except those
directly due to our acts or omissions and that your responsibility for injuries or damage to persons or
property while on or about the elevators referred to is in no way affected by this agreement.
We shall not be liable for any loss,damage or delay caused by acts of government,strikes, lockouts,
fire, explosion, theft, floods, riot,civil commotion,war,malicious mischief,act of God or by any cause
beyond our reasonable control and, in any event, we shall not be liable for consequential damages.

We shall indemnify and hold harmless the owner, general contractor and architect from any
and all damages or claims for damages which they, or any of them, may sustain by
reason of injury or death to persons or damage to property by reason of accidents
occurring during the performance of the work and caused by the act or omission of
Otis Elevator Company.
We further agree that we will carry Workmen's Compensation or Employer's Liability
Insurance fully covering our legal liability on account of accidents to our
employees. We will also carry adequate Public Liability Insurance covering our
legal liability on account of accidents growing out of our operations hereunder and
resulting in bodily injuries, including death, sustained by any persons or person
other than our employees. Insurance certificates indicating the coverage referred
to will be furnished upon request.

PREPARATORY WORK II
You Agree:
1. TO FURNISH, within thirty days from the date of acceptance of this proposal,sufficient data (including
guarantee of hoistway sizes and confirmation of current characteristics) to permit us, at our discretion, to
buy or manufacture all parts.
2. TO FURNISH a properly framed and enclosed legal elevator hoistway including, if necessary, its exten-
sion above the roof.
3. TO FURNISH an elevator pit of proper and legal depth below the lowest landing and, if required,suitable
drains and waterproofing.
4. TO FURNISH a properly lighted and ventilated fireproof machine room or penthouse(of sufficient size
to accommodate our equipment) and to supply proper access thereto.
5. TO FURNISH AND INSTALL necessary hoistway doors or gates together with sills and interlocks,
unless otherwise provided herein. t
6. TO FURNISH AND INSTALL supports close to the limits of the hoistway for the guide rails and
overhead sheaves or machine beams. t
7. TO DO all painting, except of elevator material.

8. TO DO all cutting of walls, floors or partitions together with any repairs made necessary thereby.

9. TO FURNISH AND INSTALL power, signal and light connections as follows:
(a) To terminal studs of controller or starter including suitable fused disconnect switch in the elevator
machine room. I
(b) To relay panel with fused disconnect switch in elevator machine room, when required.

(c) To light outlet in the hoistway.
10. TO FURNISH AND INSTALL whatever cutouts, circuit breakers, lightning arresters or other devices I
are necessary to meet local requirements.
11. TO FURNISH, at your expense during the erection of the elevator, electric power of the necessary char-
acteristics to provide illumination, operation of required tools and hoists and current for starting, testing
and adjusting the elevator.
12. TO FURNISH, if necessary, metal grating or concrete floors at the machine room level or metal grating
under the overhead sheaves.
13. TO GUARD AND PROTECT the hoistway during the time the equipment is being installed.
14. TO COMPLETE all work in such time as not to delay our work.
15. THAT WE shall not be responsible in whole or in part for any pro-rata expense of electric current, for
expenses of any nature relating to the rest of the building and other contractors' work, for any power
factor guarantee, for the premium of any bond (unless provided for herein) nor for any form of general
contractors' or owners' insurance (unless provided for herein).
16. TO INDEMNIFY and save us harmless against all liability growing out of your failure to carry out any
of the foregoing.
TEMPORARY USE OF ELEVATORS
Should any elevator be required for use before final completion, you agree to provide at your expense, if
required, temporary car enclosures, requisite guards or other protection for elevator hoistway openings, main
line switch with wiring, necessary power, signaling devices, lights in car and elevator operators together with
any other special labor or equipment needed to permit this temporary usage. You also agree to defray any
labor and material which is not part of the permanent elevator installation and which is required to provide
temporary elevator service. In addition, you agree to sign our "Temporary Acceptance" form before any
elevator is placed in temporary service, pay all costs of power and operation, including maintenance of the
equipment, and further agree that the complete elevator equipment will be left in the same condition of repair
that it was at the time it was turned over to you for temporary service. If repairs or replacements are neces-
OTIS sary to restore the apparatus to its condition at the time it was turned over to you for temporary service, you

Our performance of this contract is contingent upon your furnishing us with any necessary permission or ,
priority required under the terms and conditions of government regulations affecting the acceptance of this
order or the manufacture, delivery or installation of the equipment.
Quotations are subject to change without notice.
You agree to pay, as an addition to the price herein quoted, the amount of any tax based upon the transfer,
use, ownership or possession of the equipment, imposed by any law enacted after the date of this proposal or
imposed upon you by any existing law.
Payments shall be made pro-rata per elevator as follows:70 per cent when the machine is ready for shipment;
20 per cent when machine is in permanent position;and the remaining 10 per cent when elevator is in running
order. We reserve the right to discontinue our work at any time until payments shall have been made as
agreed and we have assurance satisfactory to us that subsequent payments will be made as they fall due.
The hoistway shall be ready and the proper current available by.ci.1fe /i / .5 3 after which we •

It is agreed that our workmen shall be given a safe place in which to work and we reserve the right to discon-
tinue our work in the building whenever, in our opinion, this provision is being violated.
Unless otherwise agreed, it is understood that the work will be performed during our regular working hours
of our regular working days. If overtime work is mutually agreed upon and performed, an additional charge
therefor, at our usual rates for such work, shall be added to the contract price.
The entire contract price, lets payments theretofore made and less the cost of completing the work, as esti-
mated by us, shall become due on the date named herein for the elevator to be in running order and shall bear
interest at the full legal rate from such date. If the work has been delayed by us, then the said due date shall
be postponed for a period equal in length to that of our delay. If you are not ready to accept delivery of the
machines and material upon the date above set forth for the hoistway to be ready and the proper current avail-
able, you shall on or before said date designate a local point where you will accept delivery. Unless you desig-
nate such point of delivery by the specified date, we may warehouse the machines and material within or
without our factory at your risk. You shall reimburse us for all costs due to extra handling and warehousing.
Neither you nor we shall be liable to the other party hereto for any loss, damage or delay caused by acts of
government, strikes, lockouts, fire, explosion, theft, floods, riot, civil commotion, war, malicious mischief,
act of God or any cause beyond your or our reasonable control and in no event shall either of us be liable
for consequential damages. Provided however, that, should loss of or damage to our material or work occur
at the erection site, you shall compensate us therefor unless such loss or damage results from our own acts or
omissions.
It is agreed that all the apparatus furnished hereunder can be removed without material injury to the free-
hold and we retain title thereto until final payment in cash is made, with the right to retake possession of the
same or any part thereof at your cost if default is made by you in any of the payments, irrespective of the
manner of attachment to the realty, the acceptance of notes, extension of time for payment or the sale, mort-
gage or lease of the premises.
We hereby guarantee the material and workmanship of the apparatus furnished by us under this contract
and we will make good any defects not due to ordinary wear and tear or to improper use or care which may
develop within one year from date of completion.
If any drawings, illustrations or descriptive matter are furnished with this proposal, they are approximate
and submitted only to show the general style, arrangement and dimensions of the machinery offered.
This proposal, when accepted by you below and approved by our authorized representative,shall constitute the
contract between us and all prior representations or agreements not incorporated herein are superseded.
