RE: APPROVE EIGHT PURCHASE OF SERVICES AGREEMENTS BETWEEN HUMAN
SERVICES AREA AGENCY ON AGING AND VARIOUS AGENCIES AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with eight Purchase of Service Agreements
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Human Services Area Agency
on Aging, and the following agencies:
Meals on Wheels
Catholic Charities Northern (two agreements)
Eldergarden, Inc.
Employment Development, Inc.
Weld County Health Department
RVNA Home Health Care, Inc.
Charles J. Connell, Attorney -at -Law
WHEREAS, terms and conditions are as stated in said agreements, and
WHEREAS, after review, the Board deems it advisable to approve said agreements,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the eight Purchase of Services Agreements between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Human Services Area Agency on Aging, and the above named
agencies be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1997, by and between the Weld
County Division of Human Resources' Area Agency on Aging, hereinafter referred to as "AAA," and
Eldergarden Adult Day Program, hereinafter referred to as "ELDERGARDEN."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase adult day care services from "ELDERGARDEN",
and
WHEREAS, the "AAA" and "ELDERGARDEN" are desirous of amending the original agreement of
January 2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "ELDERGARDEN" $15,000 for 16,500 units of adult day care for 85
persons.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated
in this Amendment.
3. The terms of this agreement amendment extend from January 2, 1997 through December 31, 1997.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1997, by and between the Weld
County Division of Human Resources' Area Agency on Aging, hereinafter referred to as "AAA," and
Catholic Charities Northern, hereinafter referred to as "CCN."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase a health conference for Hispanic elders from "CCN",
and
WHEREAS, the "AAA" and "CCN" are desirous of amending the original agreement of
January 2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "CCN" $2,500 for an "Hispanic Lifestyles Conference" targeted to low
income Hispanic elders.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated
in this Amendment.
3. The terms of this agreement amendment extend from January 2, 1997 through December 31, 1997.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT, made and entered into this 2nd day of January, 1997, by and between the Weld
County Division of Human Resources' Area Agency on Aging, hereinafter referred to as "AAA," and Catholic
Charities Northern, hereinafter referred to as "CCN."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase Hispanic elderly outreach services from "CCN", and
WHEREAS, the "AAA" and "CCN" are desirous of amending the original agreement of January 2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "CCN" $20,000 for 3,000 units of outreach to 180 isolated and homebound
Hispanic elderly.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated
in this Amendment.
3. The terms of this Amendment shall extend from January 2, 1997 through December 31, 1997.

PURCHASE OF SERVICES AGREEMENT AMENDMENT

THIS, AMENDMENT made and entered into this 2nd day of January, 1997 by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA," and Meals on Wheels, Inc., hereinafter referred to as "MOW."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home delivered meals from "MOW", and
WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of
January 2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse " MOW" $26,885 for 24,650 home delivered meals for 125 persons,
at least 20 to be minority.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise
stated in this Amendment.
3. The terms of this Amendment extend from January 2, 1997 through December 31, 1997. This
Amendment will be terminated immediately in the event of loss of funding.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AMENDMENT, made and entered into this 2nd day of January, 1997 by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA," and Charles J. Connell, Attorney at Law, hereinafter referred to as "CONNELL."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase legal services from "CONNELL", and
WHEREAS, the "AAA" and "CONNELL" are desirous of amending the original agreement of
January 2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "CONNELL" up to $7000 for legal consultation and
representation for the "AAA" Senior Legal Aide Program.
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. The terms of this Amendment extend from January 2, 1997 through December 31, 1997.
This Amendment will be terminated immediately in the event of loss of funding.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 2nd day of January, 1997 by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA" and RVNA Home Health Care, Inc., hereinafter referred to as "RVNA."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home health care services from
" RVNA ", and
WHEREAS, the "AAA" and "RVNA" are desirous of amending the original agreement of January
2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "RVNA" $21,175 for 2,377 units of home health care for 57
persons.
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. The terms of this Amendment extend from January 2, 1997 through December 31, 1997.
This Amendment will be terminated immediately in the event of loss of funding.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AMENDMENT, made and entered into this 2nd day of January, 1997, by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA," and Weld County Health Department's Nursing Division, hereinafter referred to as
"WCHD,"
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase health promotion/disease prevention
services for WCHD for older persons at risk of declining health due to isolation and irregular
monitoring of health status,
WHEREAS, the "AAA" and "WCHD" are desirous of amending the original agreement of June,
1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "WCHD" up to $2,500 for 120 hours of public health nurse
clinic time for five rural well adult clinics.
2.. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. This Amendment will be terminated immediately in the event of loss of funding.

THIS AGREEMENT, made and entered into this 2nd day of January, 1997, by and between the
Weld County Division of Human Resources' Area Agency on Aging, hereinafter referred to as
"AAA," and Employment Development , Inc. hereinafter referred to as "EDI."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase peer counseling from "EDI", and
WHEREAS, the "AAA" and "EDI" are desirous of amending the original agreement of January 2,
1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "EDI" $23,737.00 for 1,800 contact units of peer counseling for
90 persons.
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. This Amendment covers the period from January 2, 1997 to December 31, 1997.

RE: Area Agency on Aging Purchase of Service Agreements for 1997
Enclosed for Board approval are eight (8) Purchase of Services Agreements with local agencies to
provide services as specified in the Area Agency on Aging 1997 Annual Plan.
1. Purchase of Services Agreement between the Area Agency on Aging and Meal on Wheels
for $26,885 for 24,650 meals for 125 homebound elderly of Weld County.
2. Purchase of Services Agreement between the Area Agency on Aging and Catholic Charities
Northern for $20,000 to purchase 3,000 units of outreach services to 180 isolated and
homebound Spanish surname elderly of Weld County to help them gain access to needed
services.
3. Purchase of Service Agreement between the Area Agency on Aging and Catholic Charities
Northern for $2,500 to purchase a one day "Hispanic Lifestyles Conference" targeted to low
incom Hispanic elders.
4. Purchase of Services Agreement between the Area Agency on going and Eldergarden for
$15,000 to purchase 16,500 hours of adult day care for 85 frail elders.
5. Purchase of Services Agreement between the Area Agency on Aging and Employment
Development, Inc. for $23,737 to provide 1,800 contact units of outreach counseling to 90
older adults through the Peer Counseling Program of the Weld Mental Health Center.
6. Purchase of Service Agreement between the Area Agency on Aging and the Weld County
Health Department for $2,500 for 120 hours of public health nurse clinic time for five rural
well adult clinics.
7. Purchase of Service Agreement between the Area Agency on Aging and RVNA Home
Health Care, Inc. for $21,175 for 2,377 units of home health care for 57 persons.
8. Purchase of Service Agreement between the Area Agency on Aging and Charles J. Connell,
Attorney at Law for up $7,000 to purchase legal consultation and representation for the Area
Agency on Aging Senior Legal Aide Program.
