To Board of County Commissioners Date October 1, 199[6,

From John Pickle, Director, Health Department
Subject:
Amendment No. 2 to the Minimum Data Set Contract
Enclosed for Board approval is Amendment Number Two to the Minimum Dataset Contract
between Weld County Health Department and the Colorado Department of Public Health and
Environment.
In December 1995, the Board approved the contract which is funding a full-time staff member
who will collect minimum health indicator data. In May 1996, the Board approved Amendment
Number One which changed the start and stop dates of the contract. Amendment Number Two
will reduce the amount of funding by 8.6% ($4,730) from $55,000 to $40,270 because of a
reduction of federal Prevention Block funds available to the State. The Health Department was
notified earlier this year of this reduction in funding and has made budget adjustments
accordingly.
I recommend approval of this amendment.
