RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN -ACKERMAN AND ASSOCIATES, P.C.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Ackerman and Associates, P.C., with terms and conditions being as stated in
said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to the Child
Protection Agreement for Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and Ackerman and Associates, P.C., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of November, A.D., 1998.

RE: Amendment to Child Protection Agreemen for Services between
Ackerman and Associates, P.C., and the Weld County Department
of Social Services
Enclosed for Board approval is an Amendment to Child Protection Agreement for Services
between Ackerman and Associates, P.C., and the Weld County Department of Social Services.
The purpose of the amendment is to assure that reimbursement for Family Group Decision
Making services is consistent between the Core Services Funding and the Expedited Permanency
Planning Program Funding. The service rate for Family Group Decision Making Conference is
$1,875 with a partial services rate of$75.00 per hour, not to exceed $1,125.
All other contract provisions under the Child Protection Agreement remain the same.

OF SOCIAL SERVICES AND ACICERMAN AND ASSOCIATES, P.C.

This Amendment, made and entered into the 1st day of November 1998, by and between the County
of Weld, Colorado, by and through the Board of County Commissioners of the County of Weld, on behalf of
the Weld County Department of Social Services,and sitting as the Board of Social Services for the Weld County
Department of Social Services, whose address 915 10 Street, Greeley, Cdorado, 80631, hereinafter collectively
referred to as the"County"; and Ackerman and Associates, P.C., hereinafter referred to as the "Contractor."
WITNESSETH
WHEREAS, the County entered into an Agreement with the Contractor on June 1, 1998, to assist
Social Services in providing Family Group Decision Making Services for the Expedited Permanency Planning
Program, and
WHEREAS, the parties hereto now desire to amend the Original Agreement so as to better define such
respective rights and responsibilities, with the Amendments being based upon the parties'desire to facilitate
a different method of billing.
NOW THEREFORE, in consideration of the mutual promises and covenants stated herein,the parties
hereto agree to amend the Original Agreement as follows:'
Amend Exhibit A, Scope of Services, to read as follows:
1. Scone of Services
(Paragraph 3) The services provided by the Contractor will be for no more than 40 families
at a rate of$1,875.00 per each completed Family Group Decision Making Conference. In the
event a Family Group Decision Making Conference does not take place, the Contractor shall
be paid for the hours of service they have provided at a rate of$75.00 per hour, to a maximum
of 15 hours, or a total maximum of$1,125.00.The total cost for all 40 families shall not excecl
$75,000.00. Any amount in excess of these rates must have prior approval from Social
Services. Social Services agrees to pay the Contractor for refreshments at each conference at
a rate of$30.00 per conference for 40 conferences for a total ammnt not to exceed $1,200.00.

Amend Exhibit B, Payment Schedule, to read as follows:
2. Fees for Services
Social Services agrees to pay the Contractor $1,875.00 for each completed Family Group
Decision Making Conference. Payment in excess of this amount requires prior authorization
from Social Services. In the event a Family Group Decision Making Conference does not take
place the contractor shall be paid for the hours of service they have provided at a rate of
$75.00 per hour to a maxinum of 15 hours or a total maximum of$1,125.00. Social Services
agrees to pay to the Contractor for the cost of refreshments for each conference at a rate of
$30.00 for each conference. Any amount in excess of these rates must have prior approval of
Social Services.
Except as modified herein, the terms and conditions of the Original Agreement (PY9S-CPS-5) and
modifications to the Original Agreement(PY98-CPS-5),together with all attachments of the Original Agreement
remain in full force and effect.
