In connection with the application for Private Branch Exchange
service located in the Weld County Court House, this 4th day of,
Nay emkez 19 , $ it is understood that such service is subject
to re ations of the general Exchange Tariff of The Mountain States
Telephone and Telegraph Company.
The Private 3ranch switching equipment is furnished
to the subscriber for an initial minimum period of five years, in the
same location. In the event the application for service is cancelled
prior to complete insta3. .ation or where the use of such equipment is
discontinued or moved for any reason within the minimum period, termina-
tion and/or other charges as provided in the General Exchange Tariff
will be applicable.
