RE: APPROVE ABSTRACT OF GRANT APPLICATION FROM HEALTH DEPARTMENT
TO BUREAU OF CHILD HEALTH SERVICES, U.S. PUBLIC HEALTH SERVICE, AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Abstract of Grant Application
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Bureau of Child Health Services, U.S. Public Health Service, commencing October 1, 1997, and
ending September 30, 1998, with further terms and conditions being as stated in said abstract,
and
WHEREAS, after review, the Board deems it advisable to approve said abstract, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Abstract of Grant Application between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Health Department, and the Bureau of Child Health Services, U.S.
Public Health Service, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said abstract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 16th day of April, A.D., 1997.

The Department is applying to the Bureau of Child Health Services, U.S. Public Health Service
for a grant of up to $50,000.
The funds will be used to enhance and expand our educational and case management programs
for families impacted by Attachment Disorder and to provide education to the professional
community.

Enclosed for Board review and approval is an abstract of a grant application to the Bureau
of Child Health Services.
We would like to expand upon what we are currently doing with the Preservation of
Family funds which supports our Attachment Disorder Program through the PAC Grant.
Given the size and growth of our support group and realizing we have a larger problem in
Weld County than we originally thought, we would like to utilize this possible additional
funding to address this issue.
If we are successful in our application, the appropriate contracts will be forwarded to the
Board of County Commissioners for approval.
