RE: APPROVE REVISIONS TO TWO NOTIFICATIONS OF FINANCIAL ASSISTANCE
AWARDS FOR PLACEMENT ALTERNATIVES COMMISSION (PAC) AWARD FUNDS
AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Revisions to two Notifications of Financial
Assistance Awards for the Placement Alternatives Commission (PAC) Award Funds between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Social Services, and the Island Grove Regional Treatment
Center, Intensive Family Therapy Program, and First Steps of Weld County, Inc., Life Skills - Teen
Pregnancy Program, commencing June 1, 1995, and ending May 31, 1996, with further terms and
conditions being as stated in said notifications, and
WHEREAS, after review, the Board deems it advisable to approve said notifications, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex -officio Board of Social Services, that the Revisions to two Notifications of
Financial Assistance Awards for the Placement Alternatives Commission (PAC) Award Funds
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services, and the Island
Grove Regional Treatment Center, Intensive Family Therapy Program, and First Steps of Weld
County, Inc., Life Skills - Teen Pregnancy Program, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said notifications.

The RFP specifies the scope of services
and conditions of award. Except where
it is in conflict with this NFAA in
which case the NFAA governs, the RFP
upon which this award is based is an
integral part of the action.
Special conditions
1) Reimbursement for the Unit of
Services will be based on an hourly
rate per child or per family.
2) The hourly rate will be paid for
only direct face to face 'contact
with the child and/or family or as
specified in the unit of cost
computation.
3) Unit of service costs cannot exceed
the hourly, monthly, and yearly
cost per child and/or family.
4) Rates will only be paid on approved.

Enclosed for Board approval are revisions of Notifications of Financial
Assistance Awards (NOFA), which are for the period of June 1, 1995 through May
31, 1996.
Based on the Placement. Alternatives Commission recommendations, the purpose of
these revisions are to:
1. Decrease the yearly budget of island Grove Regional Treatment.
Intensive Family Therapy Program from $62,345.04 to $44,008.04. The
difference will he $18,337.00
2. Decrease the hourly rate of First Steps of Weld County, Inc. Life
Skills - Teen Pregnancy Program from $20.22 to $18.41. The
difference will he $1.81.
These revisions do not change any of the other original obligations or
provisions.
