HomeMy WebLinkAbout981008.tiff RESOLUTION RE: APPROVE TWENTY-THREE FY98-99 NOTIFICATIONS OF FINANCIAL ASSISTANCE AWARDS FOR FAMILIES, YOUTH AND CHILDREN COMMISSION FUNDS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with twenty-three FY98-99 Notifications of Financial Assistance Awards for Families, Youth and Children Commission Funds between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and various agencies as listed on the Department of Social Services memorandum attached hereto and incorporated herein by reference, commencing June 1, 1998, and ending May 31, 1999, with further terms and conditions being as stated in said awards, and WHEREAS, after review, the Board deems it advisable to approve said awards, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the 23 FY98-99 Notifications of Financial Assistance Awards between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and various agencies as listed on the Department of Social Services memorandum attached hereto, be, and hereby are, approved. Harbert, Chair June 1, 1998 Page 5 Intensive Family Therapy The Youth Passages IFT program will consist of two to four hours per week per family. The treatment model will be based on a brief therapy solution oriented model with an average length of treatment of 8 to 10 weeks. Parenting classes and a multifamily group are offered as adjunctive therapies at no extra charge. Home visits will be considered on a case-by-case basis. 60 total number of clients under the age of 18,five monthly average capacity,two to four hours per week per family, for 8 to 10 weeks. Hourly rate per unit of service is $70/hr. Weld County Department of Social Services Generic Day Treatment The ability to purchase day treatment for children who are placed in their own homes,family foster homes,foster-adoptive,adoptive homes,group homes, and CPAs. Five children(5-17 years) annually,five monthly average capacity, an average of five hours per week for 52 weeks will preserve placement and allows at risk children to remain in their own homes or at the least restrictive,most family-like setting; and this service program reduces placement failures and multiple moves for these vulnerable children. Monthly rate per unit of service is$1,300. Generic Sexual Abuse Treatment A maximum of ten clients will be served. The primary focus of the account would be adolescent offenders and their families. Depending on the gravity of the presenting issues,there will be variability as to how much contact is required on a weekly basis. Children who screen positive, a complete evaluation would be done by a therapist from the Attachment Center. Therapist would make specific recommendations for intervention by WCHD nurse in the family's home, monthly capacity of six to eight, three to four hours per week for six to eight months. Hourly rate per unit of service is $99.06. Weld County Youth Alternatives, dba as Partners Life Skills Program The intent of this program(MAP) is to initiate relatively short-term intervention with youth and families, helping them to negotiate a crisis or transitional period of time through provision of a "respite" or "release valve" while also helping them to develop life skills to avert crisis in the future. Maximum family units to be served is 18, children ages 8-18, monthly average capacity is six, average hours per week in program nine, average stay in program 16-24 weeks. Hourly rate per unit of service is $15.39. Greeley Dream Team Life Skills Program Services would be provided to families that have a least one child(under the age of 15) who have been scheduled for one supervised visit per week. It will serve 20 families per year, a monthly average capacity of three, for three hours per week, for 24 weeks. This program offers a 24-48 hour response time. Parents would be required to participate in three programs: Supervised Visitation, Parent Support Group, and Adult Education/Life skills. Hourly rate per unit of service is $33.65. Alternative Homes For Youth Day Treatment Program provides a comprehensive, therapeutic alternative to placement that addresses behavioral psychological, family issues and academic enrichment with a strong emphasis on vocational exploration. An average of 14 youths (12-18)per year, a monthly average capacity of seven, for a minimum of eight hours of sit-based services per day, 40 hours per week for 24 weeks. Maximum monthly rate per unit of service is $1,365. North Colorado PsychCare dba Youth Passages- Family Preservation Adolescent Partial Hospitalization Program designed to address the multifaceted needs of adolescents experiencing significant emotional, behavioral, educational, interpersonal, familial problems, and adolescents suffering from a wide range of psychiatric disorders and chemical dependency. 60 adolescents (10-18) years per year, seven monthly average capacity 40 hours per week, for six to ten weeks. Monthly rate of service per unit is $3,120(day treatment); monthly rate per unit of service Int-outpatient is $937. Services will build upon primary family connections and to help parents improve their parenting ability, identify parental strengths and support their efforts to provide care for their children. The program goal is to improve both individual and family functioning. Crisis intervention services will be provided throughout the duration of the families' involvement with our agency. Maximum number of families served per year is 32 with children 0-18 years, a maximum of eight families at any given time. Hourly rate per unit of service is $93. Intensive Family Therapy The IFT program is designed to promote the safety and well being of children and their family members. Services will build upon primary family connections and to help parents improve their parenting ability, identify parental strengths and support their efforts to provide care for their children. The program goal is to improve both individual and family functioning. Crisis intervention services will be provided throughout the duration of the families' involvement with our agency. Maximum number of families served per year is 32 with children 0-18 years, a maximum of eight families at any given time. Hourly rate per unit of service is $70. A Woman's Place. Inc. Life Skills for Battered Women The project will take an intensive and multi-faceted approach including 12 weeks of 7-8 hours of contact per week for eight total family units, and eight family units monthly average capacity. Each week the client will receive four hours of individual counseling, two group sessions, including one two-hour domestic violence education and support group and option of a one or two- hour activity session for mothers and children. Hourly rate per unit of service is $27.59. Child Advocacy Resource & Education Family Advocate Program The program is a very intensive home-based intervention program. Each family would be provided with 2 to 50 hours per month of Life Skills Training. Approximately 165 clients in the 12 month program, 40-50 total family units, 35 families per month, 6-7 hours per week for approximately 38 weeks. Hourly rate per unit of service is $38.88. First Steps of Weld County. Inc. Life Skills- Teen Pregnancy Program Provide services for Pregnant Teens (10-17) and their families and High Risk Mothers with babies (0-3), will be served with the Parents as Teachers program. Eight families per year, four families monthly capacity, three hours per week of face to face contact for approximately 26 weeks. The fust session consists of establishing the setting for mediation and the second session consists in large part of mediation of short term solution focused goals between the family members, and the final mediation session in establishing a written contractual agreement between the family members as to what they will do. Hourly rate per unit of service is $97.50. Short-term Intensive Family Therapy Short term intensive therapy to a maximum of 36 families. The service offers a range on average, of six hours weekly solution focused treatment, over a two-month period. Hourly rate of service is $97.50. Home Based Intensive Service to a maximum of 24 families. The service offers a range, on average, of three hours per week of in-home services for a 20-week period, plus follow-up at six and 12 months. Hourly rate of service is $57. Intensive Family Therapy-Family Group Decision Making The Family Group Decision Making consists of two families per month (24 per year). The Family Group Decision Making model involves extensive planning with the nuclear family of the index child, coordination with all the professionals involved in the case, and consultation with individual members of the extended family followed by a mediation style meeting with the professionals and extended family. Hourly rate of service is $75. Island Grove Regional Treatment Center Intensive Family Therapy Program This program is specifically designed to assist individuals and families with their substance abuse issues. 105 family units over 12 months, 25 units active at any one time, estimated monthly program capacity 32, and estimated average stay in intensive therapy to be 25 weeks (in-home or in-clinic). Hourly rate of service is $93.50. Individualized Family Systems Sexual Abuse Treatment This program is to develop a sexual abuse treatment program that will address and treat the individual needs of the entire family. Initial assessment to determine specific treatment needs of the family. Subcontract with professionals in the field who have already established their specialty in dealing with victims, non-offending parents, and siblings, or adolescent and adult perpetrators.
