RE: APPROVE CHANGE ORDER LETTER NO. 1 TO RYAN WHITE CONTRACT
BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT, DISEASE CONTROL AND ENVIRONMENTAL
EPIDEMIOLOGY DIVISION, AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter No. 1 to the Ryan
White Contract between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Health Department, and
the Colorado Department of Public Health and Environment, Disease Control and
Environmental Epidemiology Division, commencing April 1, 1996, and ending March 31, 1997,
with further terms and conditions being as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter No. 1 to the Ryan White Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department
of Public Health and Environment, Disease Control and Environmental Epidemiology Division,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of August, A.D., 1996, nunc pro tunc April 1, 1996.

In accordance with Paragraph 8 of routing number 96-05217 of contract number
SOD9605217, (copy attached and by this reference made a part hereof)between the State
of Colorado Department of Public Health and Environment (Disease Control and
Environmental Epidemiology Division)and Weld County Health Department covering
the period of April 1, 1996 through March 31, 1997, the undersigned agree that the
maximum amount payable by the State for eligible services in Paragraph 6 is being
increased by $30,682 to a new total of$129,162 to be spent as outlined in the revised
budget outlined below. The terms and conditions of the original contract shall remain
the same.

This amendment to the contract is intended to be effective as of 08/10/96, but in no event

Subject: Change Order Letter#1 to Ryan White Contracta/'.
Enclosed for Board approval is Change Order Letter No. 1 to the Ryan White Contract between
the Colorado Department of Public Health and Environment and Weld County Health
Department covering the period April 1, 1996 through March 31, 1997.
The change order letter amends the maximum amount payable by the State for eligible services
from $98,480 to $129,162 which is an increase of$30,682. Under the provisions of the contract,
the Health Department provides case management services for HIV affected populations in
Northeastern Colorado. The Health Department also processes pass through funding to pay for
clients' health care visits through a voucher system. Northern Colorado AIDS Project (NCAP)
and the Northeast Colorado Health Department subcontract with WCHD to provide case
management services.
