RE: APPROVE STATE PER CAPITA APPROPRIATION LETTER OF AGREEMENT AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with the State Per Capita Appropriation Letter
of Agreement between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, beginning July 1, 1995, and ending June
30, 1996, with further terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the State Per Capita Appropriation Letter of Agreement between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Health Department, and the Colorado Department of Public Health
and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 19th day of June, A.D., 1995.

Your department's allocation of the State FY 95-96 Local Health appropriation is
$167,613.00. According to the terms of Contract #ADM930242 you will receive
$149,098.00, the part of the allocation that is from the State General Fund. Of this
amount 15 %, $22,364.00, must be used for Regional Services approved by the
Colorado State Board of Health. Please submit your regional services proposal as soon
as feasible.

We can advance three -fourths of the remaining General Fund amount, $95T05 -1: -00; —
when this letter has been signed by all parties. The final one-fourth, $31;68300, will
be available when we receive the reports required by contract. 2?2,21-1.°-)

Dan Gossert will notify you about the $18,515.00 that must be used according to
Federal MCH block grant requirements.
Please sign and return all copies of this letter. This letter of approval shall not be
deemed valid until it shall have been approved by the Controller of the State of
Colorado or such assistant as he may designate.

Subject: State Per Capita Letter of Agreement
Enclosed for Board approval is the State Per Capita appropriation letter for the
period July 1, 1995 through June 30, 1996.
Weld County Health Department's appropriation for this period is $167,613.00.
According to the terms of the contract, WCHD will receive $149,098.00 from the
State General Fund for local public health efforts. Of this amount, 15% or
$22,364.00 must be used for Regional Services. The balance of the contract,
$18,515.00 must be used according the Federal Maternal and Child Health block
grant requirements.
I recommend your approval of this letter of agreement.
