HomeMy WebLinkAbout982181.tiff RESOLUTION RE: APPROVE CHANGE ORDER LETTER#1 FOR HIV PREVENTION SERVICES CONTRACT AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Change Order Letter#1 for the HIV Prevention Services Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and the Colorado Department of Public Health and Environment, commencing January 1, 1998, and ending December 31, 1998, with further terms and conditions being as stated in said change order, and WHEREAS, after review, the Board deems it advisable to approve said change order, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that Change Order Letter#1 for the HIV Prevention Services Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said change order. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 9th day of November, A.D., 1998, nunc pro tunc January 1, 1998. This change order letter will provide an additional reimbursement of$3,000 to bring the total amount of the contract to $27,830 for the period January 1, 1998 through December 31, 1998. The money will be used to purchase additional educational materials and supplies (videos, pamphlets, etc.) that will potentially improve the effectiveness of the current interventions. Koleslci,Jr. I By J Clifford W. 0,'T- '1-1'?9^ 10: 19 P.0_• SET9808287 ATTACHMENT 5 Page 1 of 2 Proposal for Supplemental Funding for HIV Prevention Contact Person: Misti Aas - 970/353-0586 ext. 2344 Weld County Health Department (WCHD) is currently funded under three separate interventions for 1998; Public Awareness, At-Risk, and Prevention Case Management (PCM). WCHD is requesting an additional $3000.00 to supplement two of these interventions; At-Risk and Public Awareness. The money would be used to purchase additional educational materials and supplies that would potentially improve the effectiveness of the current interventions. WCHD has a video lending library that is utilized by teachers and other community members, as well as the HIV Educator to supplement educational presentations. The library contains many quality titles. However, most of these videos are targeting a teen audience, or a few target a low-risk adult audience to build HIV awareness. These videos work well for the current Public Information project. The video titles listed in the proposed budget target an at-risk audience, specifically women. These will be used in the 4-session risk-reduction workshops for women that are currently being presented at the Villa (a residential treatment center) in Greeley. A few of the videos target women of color which would increase cultural competence and reflect the population being reached through these workshops. The proposed pamphlets would be utilized for both the At-Risk and Public Awareness interventions. The Video Message Pens contain 6 rotating messages; Respect Yourself, Protect Yourself, Stop the Myths, Prevent HIV, Value Every Life, and Be Aware Not Afraid. These pens have proven to be very popular with people of all ages and would also be used for both interventions. Condom Lollipops are given out at health fairs, panels and other presentations (college age and above) to promote condom acceptance and use. Grafetees are "shoe bumper stickers" to promote HIV awareness among youth. The detailed budget follows on next page.
