HomeMy WebLinkAbout951037.tiffRESOLUTION RE: APPROVE COMMUNITY HEALTH INFORMATION SYSTEM PROPOSAL FROM HEALTH DEPARTMENT TO DATA ALLIANCE COMMITTEE, NATIONAL ASSOCIATION OF COUNTY AND CITY HEALTH OFFICIALS, AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Community Health Information System Proposal from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department to the Data Alliance Committee, National Association of County and City Health Officials, with terms and conditions being as stated in said proposal, and WHEREAS, after review, the Board deems it advisable to approve said proposal, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Community Health Information System Proposal from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department to the Data Alliance Committee, National Association of County and City Health Officials, be, and hereby is, approved. Establish a leadership role for a local health department (LHD) in the establishment of an integrated Public / Private Community Health Information System (CHIS). 2. Identify and develop public / private partnerships that will help increase resources for the project. The public / private alliances are to represent organizations and disciplines involved in creating healthy communities. 3. Design integrated, comprehensive health data systems including the use of electronic communications (Internet & Bulliten Boards) to improve health related processes and decision making. 4. Test a scaled down model of the system. 5. Develop an implementation plan for the over all system. Scope of Work: The scope of the work for the project period is summarizied as follows. Task 1. The state health department will identify a liaison to ensure the needs and operations of information systems at the local level are are considered at the state level in establishing health policy and health information needs. Task 2. The local health department will establish a committee representing major health organizations and other organizations and disciplines involved in creating healthy communities. Task 3. The local health department will coordinate the development of an inventory of all data systems from the local area. The description should include a listing of the variables collected, how the data are analyzed, how the results are distributed, how frequently the the reults are updated, and who uses the information and for what purposes. Task 4. The local health department and the partners on the project will collaborate to identify the data priorities and gaps in data. 951037 Task 5. The local health department and the partners will comprehensively design a community health information sysytem that integrates the existing data from the health community and devises additional data collection methods to fill identified data gaps. Task 6. The local health department will receive committments from the participating partners as to what and how the information will be shared. Task 7. The local health department and partners will develop and implement a scaled down version of of the community health information system. Task 8. The local health department and partners will develop an implementation plan for the overall integrated, comprehensive community health data system. Tasks involving the establishment of the committee and inventory of data systems would most likely occur during the first two years of the project. System design, testing and implementation would most likely occur during the third year. Project funding provides for system hardware and software. 951037 Community Health Information System Proposal Submitted by: Weld County Health Department 1517 16th Avenue Court Greeley, CO 80631 Submitted to: Data Alliance Committee National Association of County and City Health Officials 440 First Street, NW Washington, DC 20001 951037 PROPOSAL NARATIVE Problem or Need to be Adressed The anticipation of the coming of the new mellinium presents a major opportunity for change nad has motivated as never before the bringing together of coalitions at the community level to reflect on their desire for the future and what constitutes a healthy community in its broadest context. This has had the effect of increasing citizen awareness and expectations from government agencies and local institutions at all levels and in all sectors relative to providing guidance through community assessment and planning. This phenomenom coincides with the "information explosion" and development of the "information super -highway" , providing important tools to assist in the decision making as we plan for our future. This motivation for change is clearly evident in the health care field including preventive medicine and public health. The land mark documents The Future of Public Health and Healthy People Year 2000 setforth the year 2000 challenges for public health and preventive medicine and asseessment and surveillance, including community health information systems, were given the hihgest of priority. Weld County Health Department and Northeast District Health Department have responded by centralizing the health data collection, surveillance and analysis functions with in their respective departments and have designated staffing accordingly to focus their efforts in that rergard. Both departments are in the process of upgrading their computer hardware and software to state of the art client server systems which will include data collection, surveillance and analysis capabilities. The project desrcribed herein proposes to enhance our already ongoing data collection, surveillance and analysis system by building the foundation for a comprehensive partnership between public health and the private sector through the application of electronic on-line communications technology. The resulting integrated community health information system would provide for the timely dissemination of health related data. Qualifications of Applicant Agencies Applicant Agencies and Project Participants Weld County Health Department and Northeast Colorado Health Department are local public health jurisdictions organized under authority of Colorado State Statutes and are accountable to their respective County Boards of Commissioners. The respective local health departments serve the seven northeasternmost counties in Colorado, commonly referred to as the "High Plains". The counties serve a population in excess of 200,000 people and encompass over 11,000 square miles. The area is characterized by its rural nature and agrarian economy but, within the past several years, has experienced substantial growth. 951037 Weld County Health Department, the lead agency for this proposal , was organized in 1937 ( the second oldest health department in Colorado) and has a population of approximately 135,000 people and 35 incoporated towns and villages. The broad based support for this project is evidenced by the diversity of agencies and organizations submitting letters of willingness (Attachment 1) to participate in in the demonstration project. Undoubtedly, participants in the project will increase significantly during the inventory phase of the proposed project. 2. Capacity to Covene Key Community Players and Effect Change Through Coalition Building. Past and ongoing collaborative efforts within the local health jurisdictions can be readily demonstrated at a variety of local levels as well as on a regional basis. Both agencies were instrumental in the formation of the High Plains Consortium, a coalition which brought together public health agencies and academia serving Northeastern Colorado. Members of the consortium included representatives from the University of Northern Colorado Master of Public Health Program, Colorado University Scool of Health Sciences Master of Science in Public Health Program and, Colorado State University Environmental Epidemiology Program as well as the eight locla health departments in Northeast Colorado. The Consortium has been successful in securing Funding from the Kellog Foundation which supports numerous projects including community health assessments and the support of graduate interns in public health agencies. Weld County Health Department's ability to effective convene community coalitions are summarized throughout the the attached document Weld County Health Objectives fro the Year 2000: 1994 Review . More specifically, the the Community Based Education and Health Promotion Section of the document reviews the department's ongoing efforts to build and maintain community based coalitions and advisory committees many of which were initiated based on the concerns of interested citizens in response to identified health problems. Weld County Health Department was also insrumental for convening the Weld County Community Health Coalition, a committee of providers of community health and social service agencies in Weld County. Representation includes Northern Colorado Medical Center, Sunrise and SaLud which are federally funded uban health clinics, United Way, the Area Agency on Aging ans well as several social service providers. Northeastern Health Department's collaborative abilities are perhaps, best exemplified by the regional approach to the delivery of public health services. The small population base in each of the local health jurisdiction's counties combined with the large geographic area covered necessitate partnerships at all levels of preventive health care including numerous multi -county coalitions as well as community based coalitions. 951037 3. Communities Willingness to use Health Information Within the Context of Health Planning. Without doubt, both local health jurisdictions, Weld County Health Department and Northeast Colorado Health Department, serve as the central and primary source of community health information to their citizens, allied health agencies, county and city governments as well as the numerous school based and community based coalitions desribed above. Likewise, allied health agencies, agencies of local government, community health providers and institutions share with the departments data relative to utilization and outcomes specific to their activities. The goal of the demonstration project is, in part, to formalize and increase the effiency and effectiveness of this process. 4. State Health Department Support The Colorado Department of Public Health and Environment, Health Statisitics Section, supports the proposed demonstration project as evidenced by the letter of committment included in Attachment 1. The State Health Department has been most suppotive in all of our assessment and evaluation efforts and is an important resource and ally in our preventive health care and health promotion effotrs. 5. Experience in Evaluating Public Health Programs and Information Systems The departments' experience in evaluating public health progams and information systems and analyzing surveillance systems is demonstrated, in part, by the experience and professional and educational background of its key project staff included in Attachment 2. The document Weld County Health Objectives for the Year 2000: 1994 Review demonstrates product from that experience. 6. Electronic On-line Communication Cababilty Weld County Health Department recently gained access to Internet through through contract with Colorado Supernet. The department is also logged onto CDC Wonder and is experienced in using a variety of additional on-line services and databases available through state and federal agencies. Northeast Colorado Health Department is planning on installing the reqiured hardware Scope of Work Task 1. The Colorado Department of Public Health and Environment supports the proposed project and has indicated a willingness to identify a liaison with the project and to ensure that needs and operations of information systems at the local level are considered at the state level in establishing health policy and health information needs. Project staff representing Weld County Health Department and Norteast Colorado Health Department will establish a committee representing major health organizations and other ogranizations and disciplines involved in creating healthy communities. Project staff will coordinate the development of an inventory of all data systems from the local area including data from the respective health departments, the private sector, community hospitals, social and human service organizations, schools, and community based health organizations. The inventory will include a listing of variables collected, how the data are analyzed, how the results are distributed, how frequently the systems are updated, how the results are distributed, and who uses the information and for what purposes. (completion date : year 1) Task 4. The project staff and partners in the project will collaborate to identify data prioities and the gaps in data. (completion date: year 1) Task 5. The project staff and partners on the project will comprehensively design a community health information system that integrates the existing data from the health community and devises addition data collection methods to fill identified data gaps. This system will include information on what and how data are to be collected, hardware and software needs, and how these data are to be used. (completion date: year 2) Task 6. As part of the of the overall project and as a preparatory step to implementing the comprhensive, integrated data system, Weld County Health Department and Northeast Colorado Health Department will receive committments from participating partners as to what and how the data will be shared. These committments will be outlined in memoranda of agreement and will address such issues as confidentiality and suppression of data. (completion date: year 3) Task 7. Project staff and and the partners will test a scaled down model of the comprehensive, integrated community health data system and will produce two to three examples of reports.
