RE: APPROVE CHANGE ORDER LETTER#2 FOR CASE MANAGEMENT SERVICES
AND AUTHORIZE CHAIR TO SIGN - COLORADO DEPARTMENT OF HEALTH CARE
POLICY AND FINANCE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#2 for case
management services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and the Colorado Department of Health Care Policy and Finance, commencing
July 1, 1998, and ending June 30, 1999, with further terms and conditions being as stated in
said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#2 for case management services between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and the Colorado Department of
Health Care Policy and Finance be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 4th day of May, A.D., 1998.

SUBJECT: Contract with Health Care Policy and Finance

Enclosed for Board approval is an amended contract between the Division of Human Services' Area
Agency on Aging and the Colorado Department of Health Care Policy and Finance.
This contract sets the reimbursement amount for case management services to the elderly and
disabled who are appropriate for long term care services under Medicaid.
Health Care Policy and Finance has amended the original reimbursement from a maximum of$294,
668 to a new maximum of$338,552. The final amount was calculated on case load size and a per
client reimbursement rate of$838.
The terms of this amendment apply for the period of July 1, 1998 through June 30, 1999.

In accordance with Paragraphs II-1,Page 4 and 11-9, Page 7 of contract routing number 3398-1323, as
amended, between the State of Colorado Department of Health Care Policy and Financing and Weld County
Board of Commissioners by and through the Weld County Division of Human Services Area Agency on
Aging, 1551 North 17th Avenue, Greeley, CO 80631 covering the period of July 1, 1997 through June 30,
1999 the undersigned agree the first sentence of Paragraph 9, Page 7,Compensation/Maximum Payable
which reads:

Payment pursuant to this Contract will be made as earned, in whole or in part, from available
state and federal funds encumbered in an amount not to exceed $294,668 for the purchase of
the within-described services during the period, July 1, 1997 through June 30, 1998.
is hereby amended for the period July 1, 1998 through June 30, 1999 to read:
Payment pursuant to this Contract will be made as earned, in whole or in part, from available
state and federal funds encumbered in an amount not to exceed $338,552 for the purchase of
the within-described services during the period, July 1, 1998 through June 30, 1999.
The "Annual Rate"of$814 per client which appears in the first sentence of Paragraph 6 of Page 1 of
Exhibit A is hereby amended for the period July 1, 1998 through June 30, 1999 to read:

The sentences on Page 2 of Exhibit A to the contract which now read:
However, total compensation for the contract term shall not be increased under this
mechanism by more than $18,722 above the base contract amount, i.e., to a maximum total
compensation of$294,668. The maximum increase above the base contract amount
represents 362 clients above the base client figure.

However, total compensation for the contract term shall not be increased
under this mechanism by more than $35,196 above the base contract amount, i.e., to a
maximum total compensation of$338,552. The maximum increase above the base contract
amount represents 42 clients above the base client figure.
The third full paragraph on page 2 of Exhibit A which now reads:
If the average number of community-based clients served by the Contractor falls below 322
clients during any of the first three quarters of the contract term, payment to the Contractor
during the following quarter shall be decreased. The amount of the decrease shall equal
$203.50 (one quarter of the annual rate)times the figure obtained by subtracting (1) the
average number of community-based clients served during the preceding quarter from (2)
the base client figure.
Is hereby amended for the period July 1, 1998 through June 30, 1999 to read:
If the average number of community-based clients served by the Contractor falls below 344
clients during any of the first three quarters of the contract term, payment to the Contractor
during the following quarter shall be decreased. The amount of the decrease shall equal
$209.50 times the figure obtained by subtracting (1) the average number of community-
based clients served during the preceding quarter from (2) the base client figure.
This amendment to the contract is intended to be effective as of July 1, 1998, but in no event shall it be
deemed valid until it shall have been approved by the State Controller or such assistant as he may
designate.
