RE: APPROVE REVISION TO NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR
FAMILIES, YOUTH AND CHILDREN COMMISSION FUNDS AND AUTHORIZE CHAIR
TO SIGN -ACKERMAN AND ASSOCIATES, P.C.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Revision to the Notification of
Financial Assistance Award for Families, Youth and Children Commission Funds between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Social Services, and Ackerman and Associates, P.C.,
commencing June 1, 1998, and ending May 31, 1999, with further terms and conditions being as
stated in said revision, and
WHEREAS, after review, the Board deems it advisable to approve said revision, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Revision to the Notification of
Financial Assistance Award for Families, Youth and Children Commission Funds between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Social Services, and Ackerman and Associates, P.C.,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said revision.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of November, A.D., 1998, nunc pro tunc June 1, 1998.

RE: Revision to the Notification of Financial Assistance Award
(NOFAA)between Ackerman and Associates, P.C., and the Weld
County Department of Social Services
Enclosed for Board approval is a revision to the Notification of Financial Assistance Award
(NOFAA) between the Weld County Department of Social Services and Ackerman and
Associates, P.C.
The purpose of the revision is to assure that reimbursement for Family Group Decision Making
services is consistent between the Core Services Funding and the Expedited Permanency
Planning Program Funding. The service rate for Family Group Decision Making Conference is
$1,875 with a partial services rate of$75.00 per hour, not to exceed $1,125.
All other contract provisions under the NOFAA remain the same.

Computation of Awards Description
Unit of Service The issuance of the Notification of Financial Assistance
Award is based upon your Request for Proposal (RFP).
