Home My WebLink About961168.tiff RESOLUTION RE: APPROVE CHANGE ORDER LETTER NO. 1 FOR IMMUNIZATION CONTRACT BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, DISEASE CONTROL AND ENVIRONMENTAL EPIDEMIOLOGY DIVISION, AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Change Order Letter No. 1 for the Immunization Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and the Colorado Department of Public Health and Environment, Disease Control and Environmental Epidemiology Division, commencing January 1, 1996, and ending December 31, 1996, with further terms and conditions being as stated in said change order letter, and WHEREAS, after review, the Board deems it advisable to approve said change order letter, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Change Order Letter No. 1 for the Immunization Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Health Department, and the Colorado Department of Public Health and Environment, Disease Control and Environmental Epidemiology Division, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said change order letter. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 3rd day of July, A.D., 1996, nunc pro tunc January 1, 1996. This change order letter will add an additional $12,460 to the bring the total amount of the contract to $44,381 for the period January 1st through December 31st, 1996. The additional funds will allow the Nursing Division to hold Saturday immunization clinics in the Windsor, Johnstown/Milliken, Eaton, Platteville and Ft. Lupton communities. It will also pay for posters to advertise up-coming clinics, printing for reminder cards to be sent to parer* notifying them that their children are due for immunizations, camera equipment to make photo immunization ID cards for children, computer programming necessary to upgrade the "Shots for Tots" tracking system and other operating supplies.
