RE: APPROVE CONTRACT RENEWAL LETTER#1 FOR MINIMUM DATA SET
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute arid the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter#1 for
Minimum Data Set Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
March 1, 1999, and ending December 31, 1999, with further terms and conditions being as
stated in said renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Renewal Letter#1 for Minimum Data Set Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 10th day of February, A.D., 1999, nunc pro tunc March 1, 1999.

Contract Renewal Letter Nc. 1
In accordance with Paragraph 7.of the contract with contract routing number 9808182 and contract encumbrance number
ADM9808182, hereinafter referred to as the"Original Contract"(a copy of which is attached hereto and by this reference
made a part hereof)between the State of Colorado,Department of Public Health and Environment,(Center for Health &
Environmental Information& Statistics) and the Weld County Health Department,the parties hereby agree that the
Original Contract is hereby renewed for the period of March 1, 1999,through December 31, 1999. The parties agree that
the maximum amount payable by the State for the eligible services during this renewal period is Fifty-three Thousand Four
Hundred Dollars, ($53,406.00)for a new total not to exceed amount of ONE HUNDRED THREE THOUSAND SIX
HUNDRED SEVENTY DOLLARS,$103,670.00,according to the: "Budget",which is incorporated herein by this
reference, attached hereto, and made a part hereof as"Attachment I"; and, "Scope of Work",which is incorporated herein
by this reference,attached hereto,and made a part hereof as "Attachment 2". The terms and conditions of the Original
Contract are hereby reaffirmed and shall continue in full force and effect throughout this renewal period unless otherwise
stated.
This amendment to the Original Contract is intended to be effective as of March 1, 1999,but in no event shall it be deemed
valid until it shall have beer approved by the State Controller or such assistant as he may designate.
Please sign, date and return all 4 originals of this Renewal Letter as soon as possible to: Robert O'Doherty,Division

Development of the Colorado Health Information Dataset (CoHID)
Preventive Health and Health Services Block Grant
Weld County Deliverables for 1999
The FFY-99 Preventive Block Allocation for Development of the Colorado Health Information
Dataset contains five goals. Weld County has no deliverables under goals 1, 4, and 5. Weld
County's deliverables under goals 2 and 3 are listed below.
Goal 2. Transfer the computer code and data that comprise the existing CoHID data base
from the Weld County computer system to the computer system of the Colorado
Department of Public Health and Environment (the state).
Objective 2d. Define the data mapping from the current CoHID data base to the state
data base and load the current CoHID data base on the state server by
March 31, 1999.
Objective 2f. Complete data extraction programs to routinely transfer current CoHID
data sets from suppliers to the state data base server by June 30, 1999.
Objective 2i. Provide a mid-year report of program documentation using state supplied
documentation standards by June 30, 1999.
Objective 2j. Reprogram current CoHID queries using state supported software by
December 31, 1999.
Objective 2k. Provide an end of year report of program documentation using state
supplied documentation standards by December 31, 1999.
Goal 3. Enhance the existing CoHID data set with additional health data.
Objective 3a. Add new data(e.g., 1998 data) to the existing CoHID data tables within
three months of its availability throughout 1999.
Objective 3b. Locate, develop and implement a new data module (tables and queries)
from an external data source (e.g., Colorado Hospital Association) by
Decmber 31, 1999.
Objective 3c. Locate, develop, and implement a new data module (tables and queries)
from a CDPHE data source (e.g., BRFSS survey data) by December 31,
1999. CDPHE has the responsibility to provide design specifications.
Weld County will implement the tables and the queries.

Renewal Letter#1 to Minimum Data Set Contract
Enclosed for Board review and approval is Renewal Letter#1 to the Minimum Data Set Contract
between Weld County Health Department (WCHD) and the Colorado Department of Public
Health and Environment.
This is renewal funding for the fourth year of this program. The grant will fund a full-time staff
member who will collect minimum health indicator data to share with organized health
departments throughout the State for the purposes of measuring health problems and for program
planning.
For these services, the Health Department will receive a sum not to exceed $53,401) for the time
period March 1, 1999 through December 31, 1999.
I recommend your approval of this renewal letter.
