RE: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT AND
AUTHORIZE: CHAIR TO SIGN - ELDERGARDEN ADULT DAY PROGRAM
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Human Services Department, and
Eldergarden Adult Day Program, commencing July 14, 1999, and ending December 31, 1999,
with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement Amendment between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Human Services Department, and Eldergarden Adult Day
Program be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 11th day of August, A.D., 1999, nunc pro tunc July 14, 1999.

SUBJECT: Amendment to Eldergarden, Inc. Purchase of
Service Agreement
Enclosed for Board approval is an amended purchase of service agreement between the Weld
County Area Agency on Aging and Eldergarden, Inc. Adult Day Program. This amendment will
increase funding for on-site respite services from $15,000 to $25,000 and fund an additional
$4,000 for 333 hours of in-home respite care.
The total award to Eldergarden, Inc. for FY 1999 will be $29,000.
The purpose of the amendment is to increase respite services for caregivers which has been
determined by the Area Agency on Aging Needs Survey as a significant need for Weld County.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS AGREEMENT,made and entered into this 14 day of July, 1999,by and between the Weld County
Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA," and Eldergarden
Adult Day Program, hereinafter referred to as "ELDERGARDEN."
WITNESSETH:
WHEREAS,the "AAA"' desires to continue to purchase adult day care services from "ELDERGARDEN",
and
WHEREAS, the "AAA" and "ELDERGARDEN" are desirous of amending the original agreement of
January 2, 1999,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "ELDERGARDEN" $25,000 for 16,500 units of on-site adult day care
for 85 persons.
2. The"AAA"will reimburse"ELDERGARDEN" $4,000 for 333 hours of in-home respite care to be
offered as a supplementary service when participants and families can benefit from in-home respite
or when on-site day care is not appropriate for a participant.
3. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated
in this Amendment.
4. The terms of this agreement amendment extend from July 14, 1999 through December 31, 1999.
