Greeley has a total weighted score of 97.19%, as shown on the enclosed Scoring Report. The report also includes the grade for each indicator and the grades of indicator components. Congratulations! Greeley is designated a High Performer based on an outstanding PHMAP score of 90.00 or better with no individual indicator scores below"C". Mr. Joseph Garcia, the Rocky Mountain Area Secretary's Representative,joins me in commending your accomplishment. This honor gives Greeley national recognition. Greeley may appeal its PHMAP rating based on data errors or highly unusual circumstances that occurred after submission of the Certification and/or denial of modification and exclusion requests. An appeal must provide documentation not previously submitted. Please refer to the PHMAP Handbook 7460.5, Chapter 8, for guidance. Office of Public Housing must receive the appeal by close of business 15 calendar days from the date of this letter. Greeley is granted exclusion of the following indicators: No. 7, Resident Services and Community Building No. 8, Security The Office of Public housing adjusted Greeley's score accordingly. ��� Qgtrdo 991037 Greeley's total weighted Modernization score is 10.00 points, or 100.00 percent. The modernization indicator assesses performance in carrying out the modernization program. The score helps determine the extent of OPH monitoring required and the level of any future modernization funding. A PHA which achieves a modernization score of 90 percent or higher and completed a modernization program within the last two years may be designated a High Performer for Modernization and given certain administrative relief. We reviewed your modernization program along with other PHMAP indicators and determined that we would award I:he designation, making Greeley eligible for modernization program incentives.
