The original grant application and contract was processed through the Board of Commissioners and covered years 1998 and 1999. The pages you are being asked to sign are confirmation of any changes that might have occurred in our general scope of services since that application was submitted in late 1997. A new contract will be forthcoming which will be processed through the Board of Commissioners, probably in December. Lost Community Center Board Contract ($350,000) , Gained 5310/11 ($18,580) b) Have any actual 1998 or estimated 1999 amounts increase or decreased by more than 15%? NO YES x If yes, explain on an attached page, marking response as Part II b. The net loss/gain is approximately 25%. c) Will you be able to match the funds we're providing? NO YES x Briefly describe the source(s) and amounts of match for your grant, including the additional Section 5311 funding you're receiving for 1999, using the attached page, marking your response as Part II c. SECTION 5311 GRANTEES ONLY: Please review pages 5 and 6, Tables H-2 and I-2 (Administrative and Operating budgets) in your 1997 grant application. d) Will your total 1999 administrative and/or operating amounts change by more 15%? The decrease as explained above will not have any impact on service to general public and elderly and disabled as pertaining to 5310/11 scope of service. PART III CAPITAL EOUIPMENT DATA If you are receiving funds with which to obtain capital equipment, submit an updated current vehicle inventory that includes: current vehicle mileage; current vehicle condition; any additions or deletions in the fleet. You may write the changes on your 1997 inventory, including any vehicles you have disposed of or added. If applicable, indicate the status of any vehicles that will be rehabilitated with 1999 funds. 1999 Application Update, page 2 PART IV SERVICE DESCRIPTION CHANGES Have there been any significant changes since the 1998/99 application in: a) current service description (page 12 in the 1998/99 application) NO x YES If yes, explain briefly on an attached page marking your response as Part IV a. b) Proposed project (page 13 in the 1998/99 application) NO x YES If yes, explain briefly on an attached page marking response as Part IV b. Section 5311 recipients should pay particular attention to this response, given the increase in funding and the fact that the increase might result in a Services Description that's different from your 1998 contract. c) SECTION 5311 recipients only: Briefly describe the net effect of the significant increase in Section 5311 funds. That is, for example, will you be increasing services? (If so, please describe.) Or, will you be reducing the local contribution? Provide response on attached page, marking it as Part IV c. We will continue to work on increased service in our demand response service to elderly and disabled. PART V ASSURANCES AND CERTIFICATIONS Attached are the standard assurances for Sections 5310 and 5311 that need to be completed for 1999. In addition, ETA is now requiring these additional certifications: a) "Certification of Restrictions on Lobbying" if'your agency receives more than $100,000 in ETA funding. b) "Certification regarding Debarment, Suspension and other ineligibility and voluntary exclusion' if your agency receives more than $100,000 in ETA funding. These certificates are included with this package IF they pertain to your agency. Please complete all enclosed certifications. As with your contracts, they must be signed by the highest ranking official of your organization (Board chairman/president for private nonprofit agencies and the highest elected official (mayor, county commission chairman) for public agencies. ATTACHMENT 4 1999 Application update SECTION 5311 APPLICANTS ONLY We, the Governing Body of Weld County Human Services Transportation Program (Applicant) in approving the submission of the attached application, certify: A. The Applicant is a private non-profit organization incorporated in the State of Colorado, or that it is a public body which has undertaken the requisite certification process to be designated as an eligible Section 5310 recipient B. The Applicant has the requisite fiscal, managerial, and legal capacity to carry out the Section 5310 project described herein and to receive and disburse Federal funds. C. The Applicant has or will have by the time of delivery, sufficient funds to purchase and operate the vehicles and/or equipment proposed under this project, as applicable. D. The Applicant assures affirmative compliance with Title VI of the Civil Rights Act of 1964 and related statutes. E. The transportation needs of elderly persons and persons with disabilities have or will be addressed by the Applicant. F. The Applicant has demonstrated and will continue to demonstrate efforts to achieve coordination with other transportation providers and users, including social service agencies capable of purchasing service. G. The Applicant will implement policies and procedures to ensure compliance with the provisions of the Americans with Disabilities Act of 1990. H. The Applicant has the requisite fiscal, managerial, and legal capacity to carry out the Section 5311 project described herein and to receive and disburse Federal funds. B. Some combination of State, local, federal and private funding sources has or will be committed by the applicant in a timely fashion to provide the required local share. C. The Applicant has, or will have by the time of delivery, sufficient funds to operate the vehicles and/or equipment purchased under this project, if applicable. D. The Applicant assures affirmative compliance with Title Vl of the Civil Rights Act of 1964 and related statutes. E. The transportation needs of elderly and handicapped persons have been addressed by the Applicant. F. The Applicant has demonstrated and will continue to demonstrate efforts to achieve coordination with other transportation providers and users. G.
